They provide excellent customer service to patients and assist in achieving appropriate care at the most convenient time. The Referral Management Coordinator will also manage the referral process at the primary care site to match CHA standards for referrals. Job-Specific Duties and Responsibilities: Assist patients in scheduling specialty appointments following CHA standards for timeliness and convenience for patients Manage referral pool through CHA's Electronic Medical Record, and provide appropriate documentation in patients' medical record Work closely with CHA Specialty Care offices and Central Referral Office to coordinate and manage internal & external referrals, including lab and imaging orders Assess and manage referrals at primary care site; ensure timely and appropriate care for each patient Using referral trends, identify areas of improvement; provide feedback to medical director and engage in process improvement projects for referrals Monitor different systems for processing referrals; ensure effectiveness and identify areas of improvement when necessary Track and close the loop on appropriate referrals using CHA tools and protocols Stay up-to-date with CHA protocols and insurance requirements. Requirements: Bachelor's degree or equivalent preferred. Excellent customer service skills, strong team player. Ability to multi-task and stay organized in a busy work environment. Ability to demonstrate independent judgment and provide direct feedback to clinical leaders. Proficient in Microsoft Office.
