TITLE: Peer Support Specialist II DEPARTMENT: Imani Center REPORTS TO: Director of African American Services REPRESENTED: No FLSA: Non-Exempt HOURS: Monday-Friday, 8:30am-5:00pm WAGE: DOE SUMMARY: The Imani/Hooper Peer Support Specialist II works as a liaison between Hooper, the Imani Center, and community A& D residential and outpatient clinical teams and community partners. The PSS II will engage in outreach activities with community partners and individuals to facilitate referrals, with a primary focus on accessing services to meet the treatment and recovery needs of African Americans. The PSS II will assist in the coordination of care and system navigation for individual clients and provide peer support to assist individual clients in developing recovery support within the African American community The PSS II supports recovering and formerly homeless African American clients of CCC programs by providing advocacy, mentoring, and other support. The PSS II works as a member of a multidisciplinary team that provides mental health and addictions outpatient treatment services. The PSS II will provide peer support to assist individual clients in developing recovery support within the African American community. Preference may be given to applicants who also hold a CADC, in which case, the PSS II may also facilitate alcohol and drug assessments, treatment planning, group and individual treatment sessions. ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Work with HDSC Multidisciplinary Team on specific objectives, wellness and recovery goals in conjunction with the clients ISSP. 2. Follow outreach plans developed with HDSC teams and Imani Center team and meet regularly to track progress on activities to engage prospective clients. 3. Travel daily, or as needed, to HDSC to meet with African American clients who may desire further treatment or other services in the African American community. 4. Complete and evaluate a treatment readiness questionnaire with identified African American clients. 5. Create and maintain a monthly database of information regarding specific clients who are admitted into the five (5) contracted Multnomah County beds for stimulant disorder. 6. Create and maintain a monthly outreach report that details specific contacts and further coordination of African Americans in the community. 7. Build relationships with community leaders, community partners, and advocacy groups serving the African American community. 8. Provide information, screening for clients on-site at identified programs in the African American community. 9. Help clients seek opportunities for further treatment, care, and services within Central City Concern and the African American community. 10. Work with Imani Center Team on specific objectives, wellness and recovery goals in conjunction with the clients Care Plan. 11. Exemplifies recovery principles in order to show by example that recovery is possible and attainable. Share their own recovery story as appropriate, and walks the road of recovery alongside the client. 12. Help clients seek opportunities for further treatment, care, and services within Central City Concern and the African American community. 13. Collaborate with A&D outpatient and residential clinical teams to assist with coordination of services, system navigation and peer support in the African American community, including support with discharge planning and follow-up care. 14. Visit, provide support and connect clients to the African American community, including places of worship, places of recreation, clinics and elsewhere. 15. Accompany and advocate for clients navigating healthcare, social, benefits, housing and other support services as necessary. 16. Maintain healthy professional and personal boundaries in relationships with clients, staff and community partners. 17. Facilitate recovery support activities in the African American community. 18. Update and appropriately document in client records in a timely manner with client interactions, appointments, and encounters. 19. Receive regular weekly supervision from the Team Lead and/or Director, seeking additional supervision whenever necessary. Minimum requirement two hours of documented supervision per month. 20. Provide transportation when necessary for clients in CCC vehicle, taxi, or public transportation to and from program related services in the community. 21. Attend clinical meetings and provide current information to team regarding clients participation in program and other activities. 22. CADC preferred: Facilitate alcohol and drug assessments, treatment planning, group and individual treatment sessions 23. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners and outside providers client confidentiality agreements, including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as to the program administrator. 24. Work within scope of training and experience and seek consultation when appropriate. DESCRIPTION OF OTHER DUTIES: 25. Relationship/trust building with staff, clients, community partners. 26. Participate in training opportunities to advance skills and knowledge in mental health and addiction. 27. Ability to learn how to respond to mental health crises in a clinically and culturally appropriate way. 28. Participate in compliance, quality assurance and quality improvement activities as directed. 29. Conduct/assist with organizing meetings and events. 30. Perform other duties as assigned. SKILLS AND ABILITIES: 1. Demonstrated ability to provide culturally specific services to African American men and women including experience with trauma-informed care and alcohol and drug treatment issues. 2. Ability to assist participants with developing healthy African American identity. 3. Ability to identify, access and coordinate other resources in the African American community that can provide added support to meet the needs of individuals who have experienced addiction. 4. Demonstrated ability to relate to individuals and families of varied ethnic, cultural and socioeconomic backgrounds, ages and living circumstances. 5. Ability to advocate for individuals, their identified families, and community groups/populations. 6. Ability to function as a team member and be flexible in attitude and have the desire to relate effectively with clients. 7. Ability to promote programs and services to the community (faith-based, funders, recovery/treatment providers, culturally specific groups, elected officials, governmental bureaucracy etc.) 8. Ability to learn and apply training instruction. 9. Knowledge of de-escalation methods or ability to be trained. 10. Conflict resolution skills. 11. Ability to review medical records and other client information. 12. Ability to communicate clearly and concisely both orally and in writing. 13. Ability to manage time and meet deadlines 14. Ability to work courteously and effectively communicate with the general public, medical personnel, corrections personnel, police, merchants, and co-workers. 15. Ability to develop and maintain positive working relationships with other CCC program staff. 16. Ability to work in an environment where people may occasionally be hostile or abusive. 17. Ability to maintain accurate records and necessary paperwork. 18. Ability to maintain strict standards of confidentiality with regard to patient information. 19. Ability to maintain a calm professional demeanor and make independent judgments. 20. Ability to transport clients in the company vehicle. 21. Ability to work evenings, weekends and/or holidays when necessary for the program. MINIMUM QUALIFICATIONS: 1. High school diploma or GED required. 2. Peer Support Specialist or Certified Recovery Mentor required. 3. CADC, CIT, or QMHA (Bachelors degree in Behavioral Sciences or a combination of at least 3 years work, education, training, or experience in chemical dependency or mental health treatment) preferred. 4. Engaged in African American culture. Knowledge of African American history and cultures. 5. Must meet CCC privileging requirements as required by FTCA. 6. Must be able to provide peer-delivered services as described in Medicaid guidelines. 7. Excellent reading, writing, and comprehension skills preferred. 8. Ability to maintain accurate records and necessary paperwork. 9. Ability to navigate comfortably with a computer system and having knowledge of how to use various types of software (Microsoft word, Excel, etc). 10. Education and experience sufficient to succeed, carry out responsibilities, and demonstrate the skills and abilities as described. 11. Must be a self-identified person in recovery from a substance use disorder or co-occurring mental health disorder, who meets the abstinence requirements for recovery staff in alcohol and other drug treatment programs (2years). Person must identify on the application or cover letter how they have lived personal experience leading to an understanding of the needs of persons with substance abuse and mental health issues. 12. Must have a high tolerance for ambiguity and change. 13. Will be required to carry an agency cell phone for work use. Cell phone will be provided. 14. Must have non-violent de-escalation training or equivalent (or complete within 60 days of hire.) 15. Must possess a current drivers license, pass a DMV background check and be designated an acceptable driver as outlined in Central City Concerns Fleet Safety Policy. Must pass an initial drivers training within 60 days of being an approved driver and continued recertification training. 16. Must pass a pre-employment drug screen, TB Test, and background check. This includes clearance by the DHS Background Check Unit. 17. Must adhere to agencys non-discrimination policies. 18.
