This is a full-time remote position. The individual holding this position will work on a 12-month schedule and will need to travel to American Falls to fulfill work-related responsibilities, including but not limited to home visits, center visits, staff meetings, and training events.
TYPICAL DUTIES AND RESPONSIBILITIES ESSENTIAL:
Advocate for children and families to ensure access to appropriate services.
Maintain and support a caseload of 11 enrolled children, ensuring individualized services and ongoing coordination with families and providers.
Conduct 90-minute home visits weekly through a combination of in-person and virtual to assigned pregnant women, infants, toddlers, and their families.
Conduct family assessments and develop individualized family goals.
Coordinate referrals to community partners and monitor service effectiveness.
Assist families in setting goals, accessing services (e.g., housing, employment, mental health), and navigating systems (e.g., child welfare, healthcare).
Carry out the Highscope curriculum and the Partner’s for a Healthy Baby curriculum for child development in the context of the home and during Playgroup experiences.
Facilitate bi-weekly Playgroup experiences and other family group activities.
Assess and document each infant/toddler’s development using input from families to create individualized support plans that promote healthy growth and family engagement.
Document and track child information in the program’s data system (e.g., family data, health/nutrition data, enrollment data, and program information data to support family and program goals.
Act as a resource and advocate for families by actively participating in IFSP meetings for assigned infants and toddlers with special needs, ensuring services align with the child's and family's strengths and goals.
Coordinate with health consultants and community partners to ensure comprehensive health services for children.
Provide support and follow-up to families navigating health care systems, addressing barriers to access such as transportation, insurance, or language needs.
Maintain policies, guidelines, and expectations to protect the confidentiality and privacy of families’ data.
Treat participants, fellow staff, and community members with positive regards Perform other duties as assigned.
Participate in an individual continuing education plan including CPR and First Aid.
