     POSITION SUMMARY

    Under the direction of the Chief Nursing Officer, you are responsible to coordinate, assess the training needs of the staff, plan and implement wound care prevention and treatment programs to meet the needs of improving resident skin related outcomes, evaluate outcomes, collaborate with education on policy and procedure development, guidelines, validate the competency of staff and serve as a role model for clinical practice.

    Maintain the Standard of Practice for Nursing as defined by the American Nurses Association Code of Ethics, Nursing Scope and Standards of Practice and Nursing’s Social Policy Statement. 

     

    ESSENTIAL JOB FUNCTIONS & RESPONSIBILITIES

    • Conducts a comprehensive wound exam utilizing known or innovative evidenced based tools, techniques or methods.

    • Provides recommendations to the interdisciplinary team for wound care prevention and treatment.

    • Collaborates with the interdisciplinary team members to integrate specific nursing interventions for a comprehensive plan of care or its revision to enhance resident outcomes.

    • Evaluates the effectiveness and the outcomes of the interventions individually and in collaboration with the interdisciplinary team.

    • Coordinates and participates in unit based skin care rounds to assist and educate staff with prevention, assessment and treatment of wounds, including but not limited to recommendation of dressings, appropriate support surfaces and related products.

    • Leads/coordinates regularly scheduled rounds to assess skin integrity and measure all wounds for the residents with the unit based staff and unit based skin champions.

    • Develops formal and informal educational programs targeting the competency needs of nursing personnel to improve practice outcomes related to risk and prevention strategies, and wound care treatment plans.

    • Serves as a role model through directly providing, guiding, mentoring, supporting and evaluating nursing care activities related to the specialty of wound care.

    • Participates in nursing orientation of staff on assessment of wounds, standard practices and quality outcome measures.

    • Identifies collaboratively the need for changes in the product formulary, incorporating evidence, clinical outcomes and cost effectiveness.

    • Develops/revises/implements evidenced based policies/practices/procedures/guidelines, best practice standards to improve practice related to wound prevention and care.

    • Participates in Center wide quality improvement, including monitoring, tracking, incidence and prevalence.

    • Serves as a clinical resource in the identification, investigation and resolution of the actual/potential risk management, quality of care or ethical issues.

    • Participates with the interdisciplinary team to develop a plan of care cognizant of sensory, psychomotor, maturational, biopsychosocial, physiological and age related deficits.

    • Excellent communication and verbal skills are required, along with the ability to teach staff nurses and team leaders how to integrate wound care prevention into daily care

    • Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator.

    Required Skills

    EDUCATION REQUIREMENTS:

    • RN License

    • B.S.N; M.S.N. preferred.

    • Certification in wound care as a Certified Wound Care Nurse (CWCN), Certified Wound, Ostomy Continence Nurse (CWOCN) or Wound Care Certified (WCC) is preferred.

    Required Experience

    EXPERIENCE:

    • Medical-Surgical or Pediatric nurse with a minimum of five (5) years clinical experience. Experience in pediatric and/or adolescent chronic /rehabilitative care preferred.

    • Three (3) years of experience required in Wound Care Must be able to prepare a lesson plan, write behavioral objectives, and teach a program using valid teaching strategies.

    • Certification in wound care as a Certified Wound Care Nurse (CWCN), Certified Wound, Ostomy Continence Nurse (CWOCN) or Wound Care Certified (WCC) is preferred.

    • Must be able to design self-instructional programs.

    • Must be able to coordinate and oversee a wound care program center wide.

    • Must be able to design a performance checklistMust be able to monitor quality assurance activities

    • Must possess computer based competency

    • Must be to evaluate, track, monitor, clinical performance and outcome measures.
