    Bienvivir All-Inclusive Senior Health (“Bienvivir”) is a community-based, patient-centered, comprehensive health care delivery system that advocates and promotes quality of life, optimum independence, dignity, and choices in a nurturing environment for frail seniors. Since 1987, Bienvivir has served the frail seniors of El Paso, Texas through the provision of the Program of All-Inclusive Care for the Elderly (“PACE”).

    PACE is a unique managed care benefit for frail seniors (referred to as participants) age 55 and older who are certified by the state as needing nursing home level care and who reside in a PACE service area. PACE programs coordinate and provide comprehensive medical and support services so that participants can remain independent and stay in their homes for as long as safely possible.

    Bienvivir is currently accepting applications for the following position:

    PHYSICAL THERAPIST

    Under the direct supervision of the PACE Center Manager the staff Physical Therapist (PT) is responsible for the management and overall operation of the Physical Therapy department to which they are assigned. The staff PT is charged with the responsibility for the provision of rehabilitation services to the participants in the form of evaluation, examination and utilization of exercise, mobility training, balance training, vestibular training and use of physical agents. The PT assesses the participants need for adaptive equipment or assistive devices. The PT works with BSHS staff and contracted providers by imparting the knowledge to staff necessary for the provision of quality rehabilitative services. The PT supervises assigned staff as they engage in the provision of PT to participants in the center/s, and when necessary at home, in the nursing home, assisted living facility or foster home.

    RESPONSIBILITIES:
    ** 1. Management/Supervisor of assigned department to include hiring, performance evaluations, corrective actions and termination of employees
    2. Assist in developing departmental goals and in planning annual budgetary needs of the department.
    3. Provides participant and family education as needed.
    4. General staff education
    5. Complete skills checklist and competency evaluations for direct care staff.
    6. Development/re-evaluation of the Plan of Care will be conducted upon MD orders (enrollment, annual, semiannual, as needed) on all participants.
    7. Direct therapy and documentation of patient care administered.
    8. Attend supervisors meetings, general staff meetings, departmental staff meetings, morning meetings and PCP meetings, and family conferences as well.
    9. Completes routine reassessments of participants within 72 hours of receipt of physician’s orders.
    10. Coordinates with caregiver in supervised living situations (foster and nursing homes) through joint meetings and telephone contact as needed
    11. Communicates participant concern/change of conditions to Interdisciplinary Team as needed.
    12. Completes initial assessment and plan of care within 10 working days of participant’s enrollment.
    13. Completes routine reassessments and plan of care by the participant’s scheduled I & A date. Semi-annually for participants on caseload and annually for all participants not currently on P.T. caseload.
    14. Conducts home safety evaluations as indicated.
    15. Completes participant’s reassessments and care plans due to changes in participant status as expeditiously as required by the participant’s condition.
    16. Coordinates PT evaluation at home and at contracted facilities as ordered for participants who cannot come in to the center.
    17. Documents in participant’s medical record as required by departmental policies and procedures.
    18. Communicates participant care changes to the IDT and the I&A meetings and at the daily Participant Care Planning meetings (focus meetings).
    19. Coordinates with PACE Center Manager and PT and other centers in an effort to standardize philosophy, goals and expected outcomes for the Physical Therapy Department thru Cross Site Meetings.
    20. Conducts monthly departmental meetings
    21. Participates in Performance Improvement Activities
    22. Participates in annual budgetary planning. Operates department and purchases supplies and equipment according to organizational policies and procedures and in accordance with approved budget.
    23. Follows all BSHS policies and procedures.
    24. Other duties as assigned by PACE Center Manager. **

    QUALIFICATIONS/REQUIREMENTS:
    ** 1. Graduate of accredited Physical Therapy program
    2. Licensed to practice in the State of Texas
    3. Bilingual (English/Spanish) preferred

    • 0-5 years of relevant experience or any equivalent combination of education and experience that provides the required knowledge, skills, and abilities required.

    • High School diploma/GED required, Bachelor degree preferred.

    • Knowledge of principles and methods for showing, promoting, and selling products or services required.

    • Knowledge of principles and processes for providing customer service required.

    • Knowledge of raw materials, production processes, quality control, and other techniques for maximizing the effective processing and distribution of material required.

    • Knowledge of company product(s) and their uses required.

    • Knowledge of the metals industry required.

    • Ability to multi-task, prioritize, and manage time effectively required.

    • Ability to effectively present information and respond to questions from groups of managers, customers, and the general public required.

    • Excellent oral and written communication skills, particularly a strong virtual presence and listening skills required.

    • Strong analytical and problem solving skills required.

    • Ability to apply mathematical concepts such as fractions, multiplication, ratios and proportions to practical situations required.

    • Ability to make sound decisions based on a combination of factors including analysis, knowledge and experience, and judgment required.

    • Ability to initiate and maintain cooperative relationships with co-workers and customers required.

    • Strong computer proficiency and working knowledge of Microsoft applications required.

    Work Environment : Office setting controlled temperature environment
