    We are currently seeking a Social & Human Service Advocate provides client services in a wide variety of fields, such as psychology, rehabilitation, and social work.  In this role, you have the ability to work in our Lancaster, PA office or from a remote home office.  They assist other workers, such as case managers, and they help clients find benefits or community services.  This position is accountable for enhancing member experience by helping the member to navigate the resources available to them.  It will also include acting as a liaison between members and internal/external resources. 

     

    KEY ACCOUNTABILITIES:

    • Identifies the type of resources and programs needed by members, and coordinates the utilization of these resources for the identified member population and case managers.  Acts as a liaison and advocate for elderly clients and caregivers as appropriate.
    • Works collaboratively with the case management team and other customer service teams throughout the organization to ensure the member's needs are being met.  Identifies at risk members that may benefit from referral to internal medical management services.
    • Regularly assesses and evaluates the effectiveness and quality of services provided by analyzing outcomes and reports.  Provides input on the evaluation of the program's overall effectiveness.  Makes recommendations for system development from a user's perspective.

     

     

    EDUCATION & EXPERIENCE:

    • HS Diploma or GED equivalent
    • Certificate or Associate's Degree in human services, gerontology or related subjects
    • Minimum 5 years' experience in a human services or advocacy type role supporting individuals
    • Experience with and understanding of Medicare and its related programs and services
    • Strong time management and organizational skills and ability to work independently with a high level of accountability
    • Effective communicator with the ability to convey thoughts and ideas through both oral and written communication
    • High level of critical thinking ability to be able to apply knowledge and past experiences to new challenges and situations
    • Flexibility and willingness to adapt to ever-changing business needs
    • Excellent computer skills including MS Word, Excel, Outlook and PowerPoint
