    Public Health Program Coordinator Apply for this job by 01/09/2019 Employer: DAKOTA COUNTY Located in: Apple Valley, Minnesota Degree Required: Bachelors The Public Health Program Coordinator in the Family Health Unit assists in integrating research and best practices into programs for childbearing women, families with young children and adolescent populations. The coordinator is responsible for delegated program assessment, planning, implementation, and coordination of projects, monitoring progress, writing reports, and working on program evaluation. The Public Health Program Coordinator will also inform and guide evidence based home visiting, surveillance, outreach, and case management programs and may carry a small caseload. The coordinator develops and implements standardized clinical pathways and develops a system to document assessment, intervention and outcomes in an electronic health record (EHR). The coordinator provides new employee orientation as well as continuous quality assurance/improvement with all family health staff. The coordinator collaborates with internal and external partners and works with other public/community health agencies and universities to deliver quality professional student experiences in public health nursing and related fields. Minimum Qualifications · Bachelor's Degree in Nursing AND · Three years community-based public health experience, public health nursing experience, home care or nursing facility and other related nursing.
