The Clinical Access Department has an exciting opportunity for a Patient Navigator. The Patient Navigator (PN) is a member of the healthcare team who helps patients navigate the healthcare system and get timely care. Navigators work with patients to identify their barriers to healthcare and connect them to the resources they may need such as financial assistance, counseling, language translation or transportation. The PN serves as a resource and liaison between patients, family members, practitioners, and clinical and administrative staff throughout the health system. The navigator is vital in providing proactive attention to patients to optimize their experience moving through their visit and promoting the continuum of care. The PN is responsible for responding to patient needs, coordinating and managing patient appointments, scheduling, check-in and check-out. The PN also serves as an early warning system in the management of service breakdowns and the related recovery process. The PN expedites services to the extent possible when there are organizational challenges that may interfere with timely provision of care, and they educate patients on the services provided by UHealth. Performs other duties as assigned. The minimum requirements include a high school diploma and three (3) years of relevant experience in a medical office or hospital setting required, preferably in a teaching hospital, in healthcare administration, social work, patient navigation/advocacy/relations, registration, admitting, health insurance and/or billing and collections. Excellent customer service, interpersonal, communications, critical thinking and problem solving skills. Ability to work independently and multi-task in a high stress environment, including planning, prioritizing, organizing, coordinating, and troubleshooting. Computer literate (EPIC scheduling and registration application experience a plus). Strong written and oral communication skills. Able to work in a team environment. Graceful under pressure and stressful situations. Any appropriate combination of relevant education, experience and/or certification may be considered.
