    ABOUT THE ROLE:

    A dynamic self-starter and problem solver, the Pandemic Health Navigator Program Coordinator supervises and directs the administrative and programmatic operations of the Pandemic Health Navigator Program (PHNP). The PHNP is a grant-funded, short-term program focused on reducing the disparities of outcomes of COVID-19 seen in our community through outreach/education, resource coordination, and contact tracing support. The PHNP encompasses two main programs: the Case Management Program and the Community Health Worker Program. An employee in this position will recruit, screen, and recommend eligible candidates to the Strategic Planning and Partnerships Manager. This position will also manage inventory and resources related to the PHNP, and will manage logistics, communications, and expenditures for related community activities. In addition, the PHNP Coordinator will develop monthly progress and financial reports with support from the Case Management Program Supervisor, Community Health Worker Program Supervisor, and finance representative. Work assignments are in-depth and may be of a broad scope requiring a substantial degree of responsibility and independence.

    THIS POSITION IS GRANT-FUNDED THROUGH DECEMBER 31, 2021.

     

    LOCATION: 3010 GRAND AVE, WAUKEGAN, IL 

    HOURS: 40 HOURS A WEEK; MONDAY-FRIDAY

    • WEEKENDS AND EVENING AS NEEDED

    SKILLS AND EXPERIENCE

    • Directs daily operation of the Pandemic Health Navigator Program.
    • Screens, recruits, and recommends eligible candidates for Case Management and Community Health Worker positions. 
    • Coordinates orientation/training for new staff.
    • Ensures staff have all technology and supplies needed to perform their jobs.
    • Manages and reports on assigned grant funds including monthly expenditure reports in compliance with grant rules to the designated finance representative.
    • With support from the Case Management Program Supervisor, Community Health Worker Program Supervisor, and Strategic Planning and Partnerships Manager, prepare and provide required site documentation and progress reports to IPHCA.
    • Ensures that all staff meet grant objectives and deliverables by monitoring activity in relevant databases.
    • Coordinates and integrates the PHNP Program with other Health Department and Community Health Center Programs.
    • Maintains a timely communication and training opportunities for staff through meetings, memos and continuing education opportunities based on identified staff competencies.
    • Provides program representation at internal and external meetings and committees and in the community.
    • Coordinates program education and outreach strategies for the community.

    EDUCATION, QUALIFICATIONS, AND CERTIFICATION:

    • High School Diploma is required. Bachelor’s degree in Public Health, business, or other related field is preferred.
    • Comfort with using technology to communicate and coordinate work.
    • Ability to communicate effectively with educationally and culturally diverse populations in a courteous and professional manner.
    • Management/supervisory experience of at least one year is required. Three years of progressively higher management experience is preferred.
    • Experience hiring staff to include screening and interviewing candidates for available positions.
    • Experience managing complex, dynamic programs that require frequent communication with multiple stakeholders and high levels of resourcefulness.
    • Experience managing inventory, resources, logistics, communications, and expenditures for various efforts.
    • Demonstrated ability to develop programs and to monitor and report on progress in accordance with funder requirements.
    • Preferred experience in public health, health education, or case management.

    Account Executive:

    • An aggressive, energetic, self-starter with an established skill set in solution and relationship sales
    • Proven history of performance in developing and maintaining strong customer relationships at the enterprise level, including competence and confidence in presenting at the Director, VP and C-level.
    • Excellent networking, presentation and communications skills
    • Excellent computer skills, including a strong familiarity with the Microsoft Platform, and understanding of technology trends and solutions within the unified communications marketplace
