Transitions Health Coach Work at home - Candidates MUST reside in Dayton, OH Anthem is known for transforming healthcare with trusted and caring solutions. Anthem is offering a new value added service, called care transitions, for Medicare Advantage members in the area. With the help of a Transitions Coach our members will be able to achieve goals they set for themselves and be better able to manage their chronic health conditions. The outcomes we anticipate are improved health and avoiding preventable hospitalization. We are looking for personable, energetic professionals with experience providing community based services with adults, to include visits to clients homes and collaboration with healthcare providers. In this role as patient empowerment facilitator, the Transitions Coach ® provides skill transfer and guidance to the patient/caregiver for effective care transitions, improved self-management skills and enhanced patient-practitioner communication. Duties will include teaching members how to manage their medications, red flags of their conditions, and how to communicate effectively with treatment providers. The Health Coach ® collaborates with patients/caregivers in four conceptual areas, or pillars. * Medication self-management: Patient is knowledgeable about medications and has a medication management system. * Use of a patient-centered record: Patient understands and utilizes the Personal Health Record (PHR) to facilitate communication and ensure continuity of care across providers and settings. The PHR is managed by the patient/ caregiver. * Primary Care and Specialist Follow Up: Patient schedules and completes follow-up visit with the primary care physician and/ or specialist physician and is prepared to be an active participant in these interactions. * Knowledge of Red Flags: Patient is knowledgeable about indicators that suggest his or her condition is worsening and how to respond. Qualifications: * Requires a Bachelors degree in social work, nursing or related field; and 3 - 5 years of related experience in Care Transitions, home visits, skills transfer, health education, or patient education; or any combination of education and experience, which would provide an equivalent background. * Ready access to reliable transportation is required as frequent travel to homes and hospitals will occur (around 25%). * Experience with discharge planning is preferred. * Care Transitions Program ® trained and certified Transitions Coach ® preferred. * Bilingual is a plus. * Excellent interpersonal skills, flexibility, and a passion for helping others are key for this position. * Requires strong organizational, time management skills, be an independent thinker. * Experience meeting with patients and families in a hospital or health care setting is a plus.
