ESSENTIAL FUNCTION
The Health Educator is responsible for identifying and communicating health-promoting services such as health education programs and presentations. This role will also spearhead the implementation of health services through partnerships with community-based organizations and providers. Primary duties may include coordinating health education programs in the community, communicating program needs, concerns, and resources; acting as a resource person in health education, applying appropriate research principles in health education; attending various organization meetings or conferences; and providing information to members eligible for case management program.
WHAT YOU’LL DO
COMMUNITY OUTREACH & EDUCATION: Make outbound attempts to program members to provide health information according to program standards. Educate members, prospective members, community organizations, and providers about Health Education programs and services. Recruit community organizations, resource people and potential participants for support and assistance in program planning. Maintain cooperative working relationships with public agencies and coordinate health education program activities with those of other departments and outside agencies and organizations. Prepare written educational materials for distribution to the community; update information as needed.
PROGRAM MANAGEMENT & REPORTING: Plan, organize, coordinate and participate in the development, administration, and evaluation of District health education and promotion programs including wellness, nutrition, physical fitness, tobacco and unintentional injuries. Assess community needs by obtaining and analyzing community data via surveys, interviews, monitoring, etc. Recommend and assist in the implementation of goals and objectives; develop logical scope and sequence plans for providing health education programs. Assess and evaluate service levels, operations and activities of assigned responsibilities; recommend improvements and modifications; prepare various reports on operations and activities.
