Job Description
The Regional Population Health Coordinator will provide ownership and execution on defined Value-Based Program Performance Metrics, care coordination and support for Privia’s at-risk patients. Primary work will include driving Population Health performance for assigned care centers, patient outreach to schedule patients for appointments, quality measure attestations, quality measure education to care centers, coding and documentation and payer attestations.
PRIMARY JOB DUTIES:
Review Value Based Care Program performance data (ie: quality, risk adjustment, patient satisfaction, patient scheduling rates) for assigned care centers and create action plans that support organizational goals
Evaluate payer and internal reports to identify physician and office trends
Build and develop collaborative relationships with assigned care centers in order to drive performance in Value Based Care Programs
Coordinate and facilitate monthly meetings with physicians and staff, as necessary.
Conduct patient outreach in accordance with HIPAA and patient privacy guidelines to support quality-related programs and ensure target goals are achieved
Prepare and present patient, provider, and care center level reports from the EMR and various payer portals to the care centers
Engage in patient EMR chart reviews to help close care gaps for a variety of payers and programs
Engage in multiple healthplan/payer portals to submit supplemental data to close quality gaps.
Collaborate effectively with internal teams to support care coordination for patients and drive metrics in Value Based Contracts
Provide quality measure training and other presentations, as needed, to care center staff and providers
Demonstrate critical thinking to determine the best way to support care centers as needed given expertise, knowledge, and strategies.
Answer and resolve business office inbound calls related to population health, care coordination, or other clinical activities
Make appropriate referrals to internal care team, payer programs, community resources, home care, and disease management programs
Accepts other duties, as required, willingly. Demonstrates follow through on assigned duties.
Qualifications
Clinical or office manager/leadership preferred
Knowledge of medical terminology and procedures, ICD-10, and CPT coding
Proficient with Microsoft Office applications including Word, Excel, and PowerPoint as well as Google Docs
Previous experience in a physician office and/or Medicare Advantage Program
Excellent organizational skills
Strong interpersonal skills
EHR experience preferred
Must comply with all HIPAA rules and regulations

Ability to occasionally position self to maintain equipment or materials including under desks or in closets
