Assistance programs include food, cash and medical benefits, child care, child support, employment first, veterans' services and general assistance for eligible Denver residents. Protection & Prevention services are provided via child, youth and adult protection, along with community-focused programming, including in-home services, designed to strengthen families and prevent future occurrences of abuse or neglect. Assistance Programs The dedicated staff at Denver Human Services (DHS) are here to listen and advise, helping people apply for public assistance programs. We can provide information about food assistance and financial assistance, as well as help for veterans and those who need long term care. DHS provides a range of other services, including help with transitional housing, medical assistance, and heating costs. About Our Job Denver Human Services is currently seeking a dynamic Outreach Case Coordinator for General Service to join the newest Human Services Division, Community Outreach and Resource Engagement (CORE)! We are looking for someone who is a passionate, and committed to the community. This position will perform assessment of the client's risks and needs based on intake information and determines the appropriate services and/or programs needed by the client at detention facilities once or more a week. CORE Division programs work together to connect customers to programs and services that will support them in their efforts to achieve self-sufficiency, and the primary focus of this role will be to provide those services to individuals and families in or facing homelessness through our General Assistance Outreach Unit. This role has infinite potential to expand and grow as the CORE division comes together and we are seeking innovative, creative leaders to join our team. The General Assistance/Outreach Unit offers a variety of services for people who are homeless or who are at risk of losing their housing. This unit travels frequently and are located throughout Denver County at a variety of sites and regularly attend community events. The Outreach Case Coordinator: Conducts intake interviews with homeless (or vulnerable) clients to determine eligibility for services, reviews client information such as applications for services, financial and/or employment history, social and family history, criminal history, and services received currently and previous. Performs assessment of the client's risks and needs based on intake information and determines the appropriate services and/or programs needed by the client. Works with clients to develop an individual service plan, including formulating goals with associated timelines and identifying community agencies and resources available, service providers and other pertinent resources. Mobilizes and provides on-site services to clients as needed, including having supplies, forms, resources, etc. readily available in an insured motor vehicle in order to quickly arrive on-scene as situations and needs warrant. Maintains and updates files by recording client contacts and coordinates and maintains records to ensure that federal, state and local requirements are met. Serves as an emergency responder on a 24/7 basis for emergencies that may involve client or their surroundings at risk. Performs other duties as assigned. Authorized Hiring Range: $18.84-$23.00 hourly About You Our Ideal Candidate will have: Experience processing 1 or more programs in CBMS (Case Benefit Management System) preferably SNAP and MED Experience with ongoing case management Experience with resource navigation and referrals We realize your time is valuable so please do not apply if you do not have at least the following required minimum qualifications: Education Requirement: Associates Degree. Experience Requirement: Two years of experience performing intake including assessing and identifying client strengths and needs and providing referrals for assistance. Education/Experience Equivalency: Additional appropriate experience may be substituted for the minimum education requirement. Licensure and/or Certification: Possession of a valid Colorado Class R Driver's License is required at the time of application.
