    Job Description

    Examples of essential duties of this position include but are not limited to:

    • Analyzes medical charts for completeness and indexes documents scanned into the EHR system. This includes placing documents into correct Document Type and, if appropriate, typing a description of the document; Performs quality assurance.
    • Performs Data Integrity duties including identity verification, correcting patient information, EHR and document corrections. 
    • Ensures complete and accurate medical records by following up on inconsistencies and omissions with the appropriate personnel;
    • Accurately reviews and reanalyzes all records to assure proper completion of all deficiencies, clearing charts from EHR when complete. Performs audits of transcribed reports, incomplete charts to ensure accuracy of EHR system and chart deficiencies.
    • Uses EHR, Scanning and Dictation systems
    • When assigned, provides customer service to those requesting medical records, which includes the public, health care providers, 3rd party vendors, medical examiners, auditors, and other departments within DPH.
    • Process and/or complete birth and death certificates.
    • May gather medical records from various medical providers and organizations when opening of a new case.
    • May process and analyze all incoming and outgoing faxes electronically by following standard HIM workflows (Ambulatory Clinics)

    The 2110 Medical Records Clerk also performs other related duties as assigned.

    MINIMUM QUALIFICATIONS Experience: One (1) year of experience, within the last five (5) years, performing one of the following duties: abstraction of medical information, coding, release of medical information or working in any healthcare setting dealing with medical records

    _Substitution: _ One (1) year of clerical experience AND completion of a comparable Community College course indicated below from a Health Information Technology Program approved by the American Health Information Management Association (AHIMA): Introduction to Health Information Technology, Organization of Health Data, Medical Terminology and Legal Aspects of Health Records, may be substituted for the required experience; OR

    Possession of a valid Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) certification may be substituted for the required experience.

    SELECTION PROCEDURES MINIMUM QUALIFICATION SUPPLEMENTAL QUESTIONNAIRE (WEIGHT: QUALIFYING): This Minimum Qualification Supplemental Questionnaire is designed to obtain specific information regarding an applicant's experience in relation to the Minimum Qualifications for this position. Applicants must also complete the official application. It is essential that applicants provide complete information in identifying their education, experience and training. The Minimum Qualification Supplemental Questionnaire will be used to evaluate if the applicant possesses the required minimum qualifications.

    SUPPLEMENTAL QUESTIONNAIRE EXAMINATION: (WEIGHT = 100%): Qualified candidates will be sent a Supplemental Questionnaire (SQ) exam that must be completed by the set date. Candidates will be evaluated on their relative knowledge, skill and ability levels in job-related areas. A passing score must be achieved on the SQ in order to be placed/ranked on the Eligible List. Candidate responses on the Supplemental Questionnaire cannot be changed once submitted. Candidates will be placed in rank order according to their final score on the SQ.  Supplemental Questionnaire will be evaluated in relations to the knowledge, skills and abilities required for this class which may include, but are not limited to: Knowledge of legal provisions governing medical record practices; daily workflow software applications; modern office methods and procedures; Ability and Skill to: analyze and abstract medical information; Understand and follow complex oral and written instructions; critical thinking; maintain complex medical records and prepare reports from such records, establish and maintain effective working relationships with physicians, other employees, patients and the general public. Requires sufficient skill in typing to complete work on a timely basis.

    ELIGIBLE LIST: A confidential eligible list of applicant names that have passed the civil service examination process will be created and used for certification purposes only. An examination score report will be established, so applicants can view the ranks, final scores, and number of eligible candidates. Applicant information, including names of applicants on the eligible lists, shall not be made public unless required by law. However, an eligible list shall be made available to public inspection, upon request, once the eligible list is exhausted or expired and referrals resolved. The eligible list/score report resulting from this civil service examination process is subject to change after adoption (e.g. as a result of appeals), as directed by the Human Resources Director or the Civil Service Commission. The duration of the eligible list resulting from this examination process will be of six (6) months, and may be extended with the approval of the Human Resources Director.

    CERTIFICATION: The certification rule for the eligible list resulting from this examination will be Rule of Three Scores. Additional selection processes may be conducted by the hiring department prior to making final hiring decisions. The department may administer additional position-specific selection procedures to make final hiring decisions.
