    Job Description

    ESSENTIAL DUTIES: 

    Under the clinical supervision of the Nurse Manager, the Registered Nurse in the Ambulatory Care Nursing specialty serves as a member of a multidisciplinary health care team, which delivers primary and urgent care services to a multicultural, multilingual, diverse client population. Essential functions include:

    • Performs timely client/patient telephone advice and triage assessment with clinical competence and judgment for urgent/drop in care;
    • Documents appropriately in electronic medical record;
    • Uses multiple EMR platforms to gather patient information;
    • Responds to emergencies as they occur in a timely fashion with clear delegation to Medical Evaluation Assistant (MEA)/Health Worker (HW) staff and reports to provider staff;
    • Performs nursing assessment, implements plan of care and arranges for follow up care for primary care patients, women, adolescents and children;
    • Acts as a team leader for different clinics or as Charge Nurse for the Health Center;
    • Delegates MEA/HW tasks to enhance patient care and clinic flow, and mentors MEA/HW staff;
    • Provides patient education on a variety of topics (e.g. pregnancy counseling, contraception, HIV, diabetes, hypertension, asthma and other specific diagnostic procedures);
    • Provides team-based medical management according to defined algorithms for patients with chronic diseases;
    • Provides panel management of patient cohorts with abnormal screening results;
    • Participates in daily Continuous Quality Improvement (CQI) activities;
    • Maintains , manages and monitors tracking list of patients with health care maintenance and chronic care problems;
    • Performs initial and continued assessment of client health status, immunization, and TB screening, based on established protocols;
    • Administers immunizations (pediatric/children/adult), medications and blood draws consistent with current guidelines and established protocols;
    • Provides client care in a manner that is both non-discriminatory and non-judgmental;
    • Communicates effectively with the client/patient, clinic staff, the public at large and departmental staff in a non-discriminatory manner;
    • Works with other members of the health care team to provide comprehensive primary care services;
    • Performs new patient orientation clinic (obtain initial health history, medication reconciliation, care history and update health care maintenance);
    • Participates in pre-clinic daily huddle, identifies high needs patients for the clinic session;
    • Supervises and trains non-licensed staff working in the clinic (e.g. Medical Evaluation Assistants, Health Workers II and III);
    • Covers clinical job duties of non-licensed staff when required by nurse manager; and 
    • Performs other related duties as assigned/required.
  • Qualifications

    Qualifications

    1. Possession of a valid permanent/temporary (including interim permit) California Registered Nurse License; AND

    2A. One (1) year of verifiable experience (EQUIVALENT TO 2,000 HOURS) within the last three (3) years as a Registered Nurse in an acute or urgent care, ambulatory care, or primary care clinical setting; OR

    2B. One (1) year of verifiable experience (EQUIVALENT TO 2,000 HOURS) within the last three (3) years as a Registered Nurse in any setting AND documentation of successful completion of a preceptorship program in the Ambulatory Care setting.

    The above minimum qualifications reflect special conditions associated with the position(s) to be filled. They may differ from the standard minimum qualifications associated with this classification.

    *NOTE:  One year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40 hour work week). Any overtime hours that you work above 40 hours per week are not included in the calculation to determine full-time employment.

    CARDIOPULMONARY RESUSCITATION (CPR) CERTIFICATE REQUIREMENT/PROVISION: Possession of a valid Cardiopulmonary Resuscitation (CPR) Certificate issued by the American Heart Association is required as a provision to employment for all positions. Eligibles who do not possess such a certification may participate in this examination process and be appointed to a position(s), but will be required to obtain the certificate within two (2) weeks of hire date and maintain validity throughout length of employment. A photocopy of the valid Cardiopulmonary Resuscitation (CPR) Certificate must be provided to the appropriate supervisor of the appointment within the allotted time frame. Failure to demonstrate/show proof of possession of valid certification within two (2) weeks of hire date and throughout length of employment may result in dismissal and/or termination of employment.
