    Job Description

    This is a Provisional position. Provisional applicants must participate in a Civil Service Examination for this job classification to be considered for permanent appointment.

    Under general supervision of the Center Director/Medical Director (class 2233) or the Primary Care Behavior Health Director (class 0931), the Clinical Psychologist performs the following duties:

    • Respond to requests for immediate or same-day interventions.
    • Participate in team huddles and other clinic meetings and proactively seeking patient referrals.
    • Interview and assess the social, emotional, physical and other psychosocial needs of patients presenting in the primary care setting, both in the moment during a “warm handoff” and at scheduled appointments.
    • Utilize standardized screening tools to help assess behavioral health functioning, including substance use.
    • Determine a psychiatric diagnosis based on the assessment of patients’ symptoms, self-report, and relevant collateral information and provide options and available treatments to the patient.
    • Provides supportive counseling and/or targeted individual or group therapy to patients with an emphasis on psycho-education, problem-solving, motivational interviewing, disease self-management and coping skills.
    • Facilitate and support transition to specialty behavioral health services where indicated.
    • Coordinate with other collateral contacts, services or agencies as indicated.
    • Collaborate with patients’ other primary care team members to coordinate care for patients with complex behavioral health and mental health needs.
    • Make referrals to care management services (i.e. ED Case Management, Primary Care Complex Care Management, etc.)
    • Provide active outreach and in-reach to patients to engage them in care on Primary Care patient referred by a primary care provider (PCP) as needed patients’ medical and/or psychosocial needs are met.
    • Conduct assessments of patients and complete 5150 evaluations and documentation
    • Conduct mandatory reporting activities by submitting reports (CPS, APS) within legally mandated timeframes and completing appropriate documentation.
    • Complete clinical documentation in the electronic medical record on a timely basis.
    • Complete billing encounter forms and ensure that procedure codes are appropriate to interventions and consistently documented.
    • Provide PCPs with feedback on patient encounters in person and through the electronic medical record.
    • Share information with PCPs that supports time-limited/targeted behavioral health interventions and reinforces their use in medical visits as appropriate.
    • Advocate on behalf of patients with agencies and community services.
    • Collaborate and meet with local community groups and governmental and social agencies as needed to provide and promote patient-centered care.
    • Work with the Behavioral Assistant (BA) to determine resource and linkage needs.
    • Maintain an awareness of patients’ cultures, seeking to provide culturally congruent care by asking about patient preferences, using interpreters when indicated, seeking to learn more about the customs and traditions of the patient’s culture.
    • Maintain an up-to-date collection of behavioral health patient education.
    • Access current and appropriate behavioral health education information online.
    • Participate in quality improvement activities, including collecting and reporting program data, as needed.
    • Confer with on- and off-site supervisor at designated intervals and when clinically indicated.
    • Provide coverage at other SFHN Primary Care clinics when needed.
    • Provide trainings to PCBHC staff and interns
    • Other duties as assigned per clinic needs.
  • Qualifications

    Qualifications

    REQUIRED

    • Possession of a valid license as a Psychologist issued by the California Board of Psychology as defined in Chapter 6.6 of the Business and Professions Code; Psychologist Licensing Law, Sections 2900-2903, 2914.
    • Twenty-four months of mental health programs experience*
    • Pediatrics experience*

    *These requirements contain Special Conditions in addition to this job class' minimum qualifications.

    DESIRABLE

    • Bilingual in Cantonese or Spanish is preferred.
    • Commitment to reducing health disparities and actively contributing to our goal to be an anti-racist organization 
    • Experience providing brief/short term, evidence-based behavioral health services in primary care or other medical settings
    • Ability to outreach to and build alliances with providers, patients, colleagues, and supervisors
    • Excellent verbal and written communication skills
    • Strong interpersonal skills and ability to create cooperative and effective working relationships with culturally diverse co-workers and patients
    • Strong organizational abilities, ability to manage a wide variety of tasks
