    Job Description

    Under general supervision, the 2587 Health Worker 3, will function as a Patient Navigator. The essential duties include, but are not limited to the following: 

    • Responsible for providing care navigation to ZSFG patients and providing warm handoffs to community programs while ensuring non-duplicate efforts
    • Meets with patients, families and caregivers to assess needs, provide education, support care transitions and participate in collaborative decision making
    • Follows a motivational interviewing paradigm to determine what treatment options are available to the patient and let the patient choose
    • Facilitates post-discharge phone calls to patients, family or caregivers to review discharge plan and ensure successful transition out of hospital including medication prescription acquisition, durable medical equipment acquisition and appointments as applicable within the assigned program
    • Accompanies patients as needed to financial, health insurance and legal resources if bandwidth allows and another provider is not already filing this role
    • Provide care coordination/transitions across inpatient setting and connection to immediate care. In some cases, the Health Worker 3 (i.e., Team Lily) will continue services into the community, including home, SRO’s, inpatient, medical respite and hospice facilities
    • Provides follow up for post-discharge patient phone-line – returning patient voicemails to clarify patients’ question/issue and route call to appropriate person (MD, pharmacist, SW, etc.)
    • Accompany clients as needed to medical appointments within and outside of CHN clinics to assure compliance with treatment plan
    • Advocate for patients by coordinating with multi-disciplinary team, which may include: case management, primary care teams, and medical and social service providers
    • Support and empower and work collaboratively with clients through diagnosis, treatment process and end of life care
    • Assess and provide culturally and linguistically appropriate client services including interpretation services
    • Maintain accurate documentation of services provided
    • Actively participate in all trainings and educational session
    • Participation in the Plan-Do-Study-Act problem solving process which may include simple data input and analysis to assess impact of care transitions interventions
    • May be assigned to supervise a small staff of employees in lower classifications.

    The 2587 Health Worker 3 performs other related duties as assigned/required.

  • Qualifications

    EXPERIENCE:

    1. Two (2) years of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of at least two (2) of the following duties:
    • serving as a liaison between targeted communities and healthcare agencies;
    • providing culturally appropriate health education/information and outreach to targeted populations;
    • providing referral and follow up services or otherwise coordinating care;
    • providing informal counseling, social support and advocacy to targeted populations;
    • escorting and transporting clients;
    • providing courier /dispatcher functions;
    • performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

    Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience for minimum qualification.

    One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

    3. Note on Electronic Health Record (EHR): The Department of Public Health (DPH) is implementing a unified Electronic Health Record (EHR) system and DPH employees must demonstrate competency in the use of the system that is appropriate for their classification as a condition of employment.
    4. Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing. Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
    5. Medical Examination/Drug Testing: Prior to appointment, at the Department's expense, applicants may be required to take a tuberculosis (TB) screening test, a medical examination and/or drug test.
