    Job Description

    Under general supervision of the Health Program Coordinator 2, the 2586 Health Worker 2 performs the following duties:

    • Provides client-friendly, culturally appropriate services for communities affected by health disparities, such as Black/African American, Latino/a/x, Asian, Pacific Islander, unhoused, immigrant and transgender communities;
    • Works closely with Department of Public Health colleagues, community-based organizations (CBOs), and other service providers to create referral connections and increase awareness of City and CBO services;
    • Conducts outreach and community engagement on behalf of the Community Referrals Network and partner CBOs;
    • Works directly with community members to provide referrals to meet their needs and follow-up to ensure their needs are met with relevant health and social services; connects community members to CBOS and services and provides information about available services;
    • Tracks information about the program, and records referrals and activities using electronic databases and systems;
    • Follows Network referral processes and makes suggestions to improve processes and strengthen relationships;
    • Contributes to maintaining and improving Network resource and referrals list;
    • Supports coordination of trainings and participates in CBO trainings on relevant health topics;
    • Provides written, in-person, and phone communications with community members and providers;
    • Supports convenings and information exchange among service providers
    • Contributes to assessment of referral quality and strengthening of referral relationships;
    • Contributes to ongoing identification and mapping of organizations that serve the city’s population
    • May support assessments of clinic and community testing/vaccine sites and suggest ways for sites to improve access to community; and
    • Provides support to community advisory groups and community coalitions.

    The 2586 Health Worker performs other duties as assigned/required. 

  • Qualifications

    EXPERIENCE: One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties: Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier/dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

    Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

    Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted. 

    One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

    DESIRABLE QUALIFICATIONS: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring. 

    • Experience working with or in a community setting, or in community-based organizations in one or more of the 9 neighborhoods
    • Knowledge of community-based organizations in one or more of the 9 neighborhoods
    • Experience working with underserved populations including racial and ethnic minority groups experiencing health disparities
    • Language skills in languages other than English desired:
      • For Liaisons working in the Mission, Excelsior, and OMI neighborhoods, fluency in Spanish is especially desirable
      • For Liaisons working in the Chinatown, Tenderloin, and Western Addition neighborhoods, fluency in Cantonese is especially desirable
    • Knowledge of services related to COVID-19, including economic recovery, food, and housing services preferred but not required
    • Understanding of public health and social services available in San Francisco
    • Skilled and able to work well in Microsoft Office Suite including Word, Excel, Outlook, and Teams
    • Comfortable using computers for MS Office, email, and virtual meetings
    • Ability to multi-task and respond to emerging challenges with flexibility

Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
    6. Medical Examination/Drug Testing: Prior to appointment, at the Department's expense, applicants may be required to take a tuberculosis (TB) screening test, a medical examination and/or drug test
