    Job Description

    UNDER GENERAL SUPERVISION OF THE OPERATION MANAGER/CHARGE NURSE/NURSE, THE 2586 HEALTH WORKER 2 PERFORMS THE FOLLOWING DUTIES IN STREET MEDICINE AND SHELTER HEALTH CLINICS:

    • Provides health education, information, and resources to patients and others regarding health and other facilities available to them; assists them in utilizing such services; makes follow-up contact required;
    • Serves as liaison between the professional staff and the community;
    • Assists in gathering and evaluation data concerning the program which assigned; may perform incidental clerical duties such as keeping records, answering the telephone, and arranging client appointments;
    • May transport ambulatory patients between their homes and clinic, hospitals, or other social agencies; may transport staff members to meetings with administration approval; reports malfunctions of the vehicle to supervisor;
    • May pick up and deliver supplies and equipment, including high-security pharmaceutical supplies, laboratory tests, and mail;
    • Provides clinical support to assigned Navigation Center/Shelter medical staff (supervising clients waiting, organizing patient flow, crowd management);
    • Performs clinical duties (e.g.check vaccine fridge temperature and record, check medication and PPE stock, take vitals and record) and collects demographic and chief complaints before being seen by clinic provider;
    • Prepares assigned clinics for operations (stocking supplies, ordering and picking up medication) and performs administrative duties as needed. 
    • Assists with tracking linkage to primary care (scheduling appointments, reminders, follow up and escorts); arranges client case conferences and in-service with community partners;
    • Practices motivational interviewing techniques with individuals to assist them in making changes to improve their health;
    • Educates individuals about interventions that may improve their health;
    • Works closely with the WPIC teams: Street Medicine, Shelter Health, Urgent Care Clinic, SORT/POET, ETE, + SF HOT case management, dispatch, outreach, and transport staff;
    • Participates in clinical and non-clinical team meetings, supervision meetings, case conferences, staff meetings, in-service training, and other staff development activities; and 
    • Develops and maintains clear, open, timely, cooperative, and diplomatic communications and working relations with all staff, consumers, and partner agencies.

    The 2586 Health Worker 2 performs other related duties as assigned/required.

  • Qualifications

    Qualifications

    1. EXPERIENCE: One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:
    • serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations;
    • providing referral and follow up services or otherwise coordinating care;
    • providing informal counseling, social support and advocacy to targeted populations;
    • escorting and transporting clients;
    • providing courier/dispatcher functions;
    • performing pre-clinical examinations of vital statistics, such as measuring a patient's weight, height, temperature and blood pressure; AND

    SPECIAL CONDITION: 

            2. Twelve (12) months of homeless programs experience

    Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience for minimum qualification #1.

    One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.) Any overtime hours that you work above forty (40) hours per week are not included in the calculation to determine full-time employment.

    Desirable Qualifications: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring.

    • Experience, and training in street outreach and healthcare for homeless individuals with complex mental health, medical, substance use, and social conditions
    • Desire to work with homeless population
    • Knowledge of community resources and experience in community settings.
    • Conflict resolution and de-escalation skills.
    • Customer service orientation.
    • Excellent problem-solving skills, analytical ability, and skills to handle complex organizational and clinical issues.
    • Certification or experience in drug and alcohol counseling
    • Excellent oral and written communication skills
    • Ability to build strong working relationships with colleagues and community partners
    • Ability to work well with diverse staff, collaborators, and patients including cultural, language, sexual identity, gender and other diversity considerations
    • Ability to work as part of a team, prioritize and handle multiple tasks and to work independently in a high-pressure environment
    • High level of accountability, with the ability to work independently and cooperatively.
    • Excellent attendance record
    • Bilingual, bicultural in Spanish or Cantonese/Mandarin.
    • Previous experience or training in harm reduction 
    • Experience and training in street outreach and healthcare for homeless people.
    • Experience with electronic health records, documentation, and billing procedures.
    • Ability to work with electronic and computer systems including word processing, spreadsheets, databases, presentation, and other relevant software

The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.
