    Job Description

    Multiple positions available! 

    • Some are limited to 1,040 work hours in a year while other positions can last up to three years
    • All are "at will" and serve at the discretion of the department head

    Medical Social Workers provide comprehensive medical social services to clients and their families; perform routine medical social work duties; evaluate social, emotional and physical needs of clients; interview clients and their families and significant others; provide supportive counseling or crisis intervention; conduct mandatory reporting activities; advocate on behalf of clients; provide discharge planning, service coordination, psychoeducation and case management. Essential job functions include:

    • Collaborates with interdisciplinary health care teams;
    • Evaluates social, emotional and physical needs of clients;
    • Conducts mandatory reporting activities by completing reports (CPS, APS, elder abuse, and domestic violence reports) in specific timeframes and completing appropriate documents;
    • Prepares records to document case activity; 
    • Provides supportive counseling or crisis intervention as needed;
    • Interviews clients, their families and significant others to obtain intake information;
    • Provides discharge planning and/or service coordination and linkage by analyzing client needs;
    • Interprets medical information to reinforce or clarify medical assessments;
    • Provides educational information by explaining health issues such as substance abuse, HIV, prenatal care or nutrition;
    • Acts as a resource for other staff by providing training and consulting on psychosocial issues to enhance treatment of clients;
    • Performs outreach and case finding to identify potential clients with psychosocial needs;
    • Integrates services or programs to increase efficiencies;
    • Reviews service referral request inquiries received by phone or computer system;
    • Performs community outreach and case finding, representing the organization at various community events;
    • Advocates on behalf of clients with agencies, medical practitioners and within the community; and
    • Conducts home visits when necessary to survey living situation of clients for accessibility and to determine the need for further services.
  • Qualifications

    Qualifications

    REQUIRED

    • Possession of a Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE); AND
    • Six (6) months (1000 hours) of social work experience in a health care setting. This experience may be gained from supervised field placement during the Master's in Social Work (MSW) program or from post-graduate social work experience.

    • Experience in inpatient, outpatient, or home health based social work

    ELECTRONIC HEALTH RECORD (EHR) The Department of Public Health (DPH) is implementing a unified Electronic Health Record (EHR) system and DPH employees must demonstrate competency in the use of the system that is appropriate for their classification as a condition of employment.  

    PERSONAL PROTECTIVE EQUIPMENT (PPE) Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e., weakens the seal between the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.  

    Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a Face Fitting Respirator.  

    MEDICAL EXAMINATION Prior to appointment, at the Department’s expense, applicants may be required to take a tuberculosis (TB) screening test, medical examination, and/or drug test.
