Community Health Worker Program Manager
Job Description
The Community Health Worker Program Manager will direct the overall operation of the Philly Counts Community Health Worker Program including overseeing the implementation of program activities, coordination with other agencies, development of materials, and the recruitment, assignment, and management of 10 Community Health Workers. This individual will be responsible for ensuring reports and documentation are submitted to the CDC in an accurate and timely fashion. This position will report to the Director of Partnerships.
This is a grant funded position and is guaranteed for a minimum of one year, with an additional two years contingent upon satisfactory programmatic progress and availability of CDC funds.
ESSENTIAL FUNCTIONS
Primary Duties: ● Develop partnerships and coordinate with government agencies, health organizations and stakeholders, and members of community-based organizations to operationalize organizing efforts ● Create operational and outreach materials used by Community Healthcare Workers ● Collaborate with Community Health Care workers to address and solve issues they experience and support them in completing daily and weekly program goals ● Verify, maintain and report program data to the CDC ● Communicate weekly project updates to Philly Counts, the Philadelphia Department of Public Health and Community Healthcare Workers ● Successful completion of Temple’s Community Health Workers Certification, participation during work hours and paid for by employer ● Lead recruitment and hiring efforts to bring on 10 community health workers ● Additional related responsibilities as needed

PROJECT MANAGER: ability to create high level strategy and outline the steps needed to meet all objectives, strong organization skills, detail-oriented, thorough in completing tasks, and persistent in pursuit of goals MISSION-ORIENTED: passionate about and dedicated to providing services to individuals living with chronic physical and behavioral health conditions in North Philadelphia CRITICAL THINKER: proven ability to work through complex problems/ tasks and the ability to prioritize and multi-task to meet deadlines ADAPTABLE: ability to understand and respond quickly to changes in the environment or direction; ability to toggle between big picture and details; able to be motivated about many different kinds of tasks and projects; self-starter with the flexibility to work independently and as part of a team EFFECTIVE COMMUNICATOR: strong oral and written communication skills, strong interpersonal skills, high level of emotional intelligence, and a track record of building effective relationships with diverse people Familiarity with electronic tools: Fluency in Microsoft Office Suite required; excellent skills in Excel and PowerPoint; experience with VAN a plus
Qualifications
• Demonstrable experience managing projects and a team of staff • Passion for advocacy and social change • Availability to work a flexible schedule, including some evenings and weekends • Passion for community organizing and social justice • An understanding of the many social determinants of health • Ability to work collaboratively and meet deadlines • Experience collecting and analyzing data and creating comprehensive reports • Proven ability to solve problems creatively • Excellent communication skills and an eagerness to serve diverse audiences, including community-based organizations, elected officials’ staff, faith communities, students, volunteers, and the general public • Prior experience community organizing and managing volunteers is highly preferred
• Bilingual in Spanish or Mandarin is a plus
Additional Information
Salary Range: $65,000

The successful candidate must reside in the City of Philadelphia or establish primary residence in the City of Philadelphia within six months of employment.
