Under general direction, is responsible for coordinating, planning and directing the delivery of case management and the analysis of programs. Conducts research and analyses on policies and procedures.  The DHS Homeless Outreach Clinical Coordinator will engage and provide outreach to persons experiencing homelessness with a severe mental illness, substance use disorder or medical concern residing in encampments, on the streets, and places not meant for human habitation. The position will be housed in the DHS Homeless Service Division; serve as a member of the Coordinated Homeless Encampment Outreach Team, work closely with the SAPD, community partners and participate in other city-wide outreach efforts. Working conditions include working inside and outside an office environment.  Exercises direct supervision over assigned staff. This position is a “grant funded” position, therefore, unclassified and has no disciplinary appeal rights to the Municipal Civil Service Commission. This position is likewise subject to at-will employment status and can be terminated at any time for any reason.

 Essential Job Functions:

• Supervises trains and directs assigned staff to facilitate effective and responsive case management to clients in grant and/or general funded social service programs and activities.

• Supervises, trains and directs assigned staff to implement social service-related programming and activities.

• Provides clinical coordination and assessments, case management to clients to assist them with housing navigation, employment and stabilizing their mental health and medical situations.

• Promotes, educates, and supports program goals and objectives.  Drives continuous improvement of social service-related programs services and activities based on current participant and community needs.

• Provides oversight of counseling services to families and individuals in social service-related programs and activities.

• Leads the design, development, implementation, and expansion of community partnerships that enhance the program's effectiveness.

• Serves as liaison between department and community boards and committees.

• Analyzes complex data, policies, practices, systems, procedures, and develops written summaries on findings and recommendations. Prepares management reports and delivers presentations.

• Monitors program performance relative to goals and objectives and takes appropriate action, when indicated, to ensure compliance with grant and program performance requirements. Monitors expenditures to ensure compliance with grant contract.

• Reviews monitoring outcomes and conducts necessary and timely follow-up and supervision for correction with appropriate staff.

• Interviews, selects, supervises, develops, evaluates, counsels, and disciplines personnel according to established COSA policies, procedures, and guidelines.

• Performs related duties and fulfills responsibilities as required.

Qualifications

• Master's Degree in Social Work from an accredited university.

• Three (3) years of experience in social work with children and families or related experience.

• Three (3) years of supervisory experience in social work with children and families.

• Licensed Professional Counselor (LPC), Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW) from the Texas Department of State Health Services.

• This position requires a valid Class "C" Texas Driver's License OR valid driver's license from another state with the ability to obtain a valid Class "C" Texas Driver's License within 30 days of becoming a Texas resident.  Use of your own personal vehicle may at time be required and proof of Liability Insurance will be required.

PREFERRED QUALIFICATIONS: 

• Two years of experience with street outreach and working with the homeless population.
