    Job Description

    Under general direction, is responsible for entry level professional community health work in under Metro Health Community Health Worker Hub as a part of the Access to Care Division. Serves as a liaison between health and social services and the community to facilitate access to services and improve the quality and cultural proficiency of service delivery and work to improve health equity. Conducts outreach to implement programs in the community that promote, maintain, and improve individual and community health. Working conditions are in a community-based environment and an office environment. Exercises no supervision.

    This position will work specifically to connect individuals exiting the justice system with essential resources and ensure their continuity of care. The ideal candidate will have experience as a Community Health Worker, experience with a criminal justice population, and working with individuals from diverse background and experience.

  • Qualifications

    Qualifications

    • High School Diploma or GED equivalent (recognized by the Texas Education Agency or a regional accrediting agency).
    • Two (2) years of experience working as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer.
    • This position requires a valid Class "C" Texas Driver's License or the ability to obtain a valid Class "C" Texas Driver's License within thirty (30) days after becoming a resident of the State of Texas. Use of your own vehicle may at times be required and proof of Liability Insurance will be required.
    • Any applicant selected for this position must have a background check completed with results satisfactory to the City of San Antonio and successfully complete the San Antonio's Cash-Handling Certification Training.

    Preferred Qualifications

    • Bilingual in English and Spanish.
    • Community Health Worker Certification.
    • Experience working with vulnerable populations.
    • Direct current and/or previous experience with reintegration from jail is strongly preferred.
    • Significant current and/or previous experience with navigation to social services, health care, behavioral health, and Medicaid applications.
    • Demonstrated commitment to engaging and serving people who have been incarcerated.
    • Demonstrated commitment to equity and respectful engagement with communities experiencing violence.
    • Experience developing and maintaining relationships with diverse community partners including healthcare centers and community agencies.
    • Experience with de-escalation and conflict mediation.
    • Ability to communicate with multiple types of audiences.
    • Ability to use good judgment and work independently without supervision.
    • Basic knowledge of criminal justice process and terminology.
    • Team player.
