    • The position(s) is/are not exempt from FLSA requirements for time and a half overtime compensation. The employee(s) will earn time and a half for time worked in excess of 40 hours per work week. Work Week : 37.5 hr/wk Pay Grade : 14 Special Entrance Rate : No Description of Job Duties This position will provide intensive case management services, coordination and oversight of the Hart-Supported Living program. In addition to providing direct services to the disabled population this position will provide counseling, advocacy and case coordination. Conducts face to face visits with recipients in the home. Processes and assists Recipients with required program documentation, including reimbursement documentation, employee and employment documentation, and services authorization information. Assists in the development of service objectives, comprehensive service plans and budgets. Compiles and maintains regional data on recipients demographic information, and other program-related documentation and report data required to be submitted to the State Coordinator. Provides direct assistance to all applicants on program applications and Recipient renewals. Manages program inquiries and acts as liaison between Recipients and all other entities. Manage the distribution, review, and acceptance of program applications and program renewals in accordance with established policy. Assist program Recipients with the development of individualized program plans. Reviews the recipients plan to ensure it is tailored to the Recipients need and budget. Processes timesheets and invoices and assures compliance with recipients plan and budget. Manages budgetary allowances per recipient monthly. Interpret policies, regulation and statutes to ensure compliance within Hart-Supported Living program. Monitor Recipients and provides case coordination according to department policy and regulation. Facilitates the development of community partnerships with participants, human services providers and employers. Coordinates and monitors provider services to avoid duplication or omission of services. Develops processes for enhancing community awareness. Participate on program application review teams and attend meetings as necessary or directed. Other duties as assigned.
