NP- Telehealth - Phoenix, AZ - Overview Summary Duties A TH NP provides professional assessment, medical management, and coordination of health care services via clinical video technology (CVT). The TH NP acts on behalf of the Veteran to ensure appropriate clinical services are received and manages a panel of HT patients using the Disease Management Protocol (DMP) to meet clinical goals.The position includes the management of labs, medications, consults and/or follow-up planning. Progress notes and orders are required on all patients. In addition, the TH NP provides comprehensive in-home visits via CVT in collaboration with local fire departments. Provides intense education and medical management of all assigned patients to prevent re-admissions and improve health outcomes. Works in collaboration with Primary Care and Specialty Care providers to optimize medical conditions. Participates in care planning and works closely with patients, families, primary care providers, allied health professionals, team members, internal/external partners, and community agencies to coordinate care. The TH NP assists the HT Team Leader in providing direction and guidance to other HT team members. Serves as a backup for other TH NPs. Participation in clinical research protocols, professional lectures/reviews and involvement on support committees and/or workgroups is encouraged.
