required. Duties The incumbent has the clinical knowledge, skills and
abilities to: Independently perform case management and outreach duties
- identifying, screening and serving patients with psychosocial needs
Independently implement treatment modalities, including the provision of
counseling or psychotherapy for individuals, families and groups.
Independently conduct psychosocial assessments and subsequently develops
treatment plans in collaboration with the veteran/family and with the
interdisciplinary treatment team. Facilitate psychosocial interventions
and measures outcomes to interventions. Coordinate community-based
services, including information and referral for additional services
from other VA programs, other government programs and community agency
programs. Effectively identify high-risk patients and provides case
management services. Provides crisis intervention services, seeking to
address the cause as well as the presenting complaint, coordinates
family conferences and serves as liaison to family members. Demonstrate
effective use of medical and mental health diagnoses, disabilities and
treatment procedures. Establish and maintain effective therapeutic
relationships with Veterans and their families. Work independently with
patients and families who are experiencing a variety of psychiatric,
medical and social problems utilizing individual, group and family
counseling and therapy skills. Independently provide consultation
services to other staff regarding the psychosocial needs of veterans and
the impact of psychosocial problems on health care and adherence to
treatment plan. Evaluate incumbents own practice through professional
peer review, case conferences and other organizational means.
Communicate effectively, both orally and in writing, with people from
varied backgrounds. Assist Veterans with general information regarding
benefits and services relating to special programs, service-connected
compensation, non-service connected pension. Facilitate referrals based
upon veterans needs and eligibility. Incumbent operates a government
vehicle as an essential function of the job and could be required to
transport patients in a government vehicle, even one time per year.The
incumbent is required to have and maintain a valid state driver's
license and to successfully complete any annual physical or education
requirements to maintain the driving requirements of the position. -
