Social Worker (HUD-VASH) - Junction City, KS - Overview Summary Duties Duties included but not limited to: - Responsible for providing case management for a caseload of veterans in the HUD-V ASH program. The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-V ASH placements. - Combing HUD rental assistance for homeless persons with case management and clinical services provided by the Department of Veterans Affairs (VA) at its medical centers and in the community. - Performing site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes. - Verification of the veteran's status, eligibility for VA medical care, and eligibility of program participation. - Ensuring that homeless veterans are considered as primary participants for program acceptance. - Collecting and submitting HUD-V ASH Program participant data as outlined by NEPEC evaluation procedures. - Providing HUD-V ASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing. - Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery , and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies. - Complies with criminal conflict of interest laws and Executive Branch Standards of conduct so as to avoid conflicts of interest while serving as the HUD-V ASH Case Manager. - Responsible for following local policy for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARP and JCAHO accreditation. - Will conduct psychosocial assessments to determine needs of veterans and family members. - Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. - Provide consultation/education to veterans and families on community resources, advance directives and VA benefits. - Will maintain data and statistical compilations to comply with JCAHO, CARF, VA and medical center policy and procedures. - Other duties as assigned. Grade Determinations: GS-11 Level: In addition to the basic requirements, must have a minimum of one year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in the state at the independent practice level. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs below: 1.Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. 2.Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team. 3.Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture. 4.Knowledge and experience in the use of medical and mental health diagnosis, disabilities and treatment procedures.This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology. 5.Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals.This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques. 6.Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families.Social workers must practice within the bounds of their license or certification.For examples, some states may require social workers providing psychotherapy to have a clinical level of licensure. 7.Ability to provide consultation services to otherstaff about the psychosocial needs of patientsand the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students.
