Virtual Care RN (Community Based Outpatient Clinics) - 3 Total Vacancies for the following locations: (1) Longview TX, (1) Monroe, LA, (1) Texarkana, AR) Work Schedule: Work Schedule (Virtual Care RN): 7:30 a.m. - 4:00 p.m. Monday - Friday. Home Telehealth RN - 1 Total vacancy for the following location: (1) Shreveport, LA Work Schedule: (Home Telehealth RN) : 7:30 a.m. - 4:00 p.m. Monday - Friday. This is a telework position. Financial Disclosure Report: Not required Duties VIRTUAL CARE RN (COMMUNITY BASED OUTPATIENT CLINICS): The RN duties of these positions include, but are not limited to: Utilizes the nursing process and evidence-based practice to work collaboratively with the Facility Telehealth Coordinator, Supervisors, clinical and administrative staff, Information Technology, BioMed and other services to provide care to patients using various TeleHealth modalities. Acts as a liaison to Providers and staff across all service lines, in developing and providing patient-driven holistic care through various TeleHealth Technologies. Provides leadership, coordination, management, implementation and evaluation of Telehealth programs such as: Clinical Video Telehealth (CVT), CVT to Home, Store & Forward TeleHealth and other programs to be developed that provide care to Veterans using interactive and real-time video conferencing technologies. Promotes an environment that embraces new Telehealth technology, innovations and improvements. HOME TELEHEALTH RN (VIRTUAL CARE): The RN duties of these positions include, but are not limited to: The RN Care Coordinator is a specialized case manager who coordinates care for a panel of patients throughout the continuum of care to assure that care is timely, appropriate, evidenced-based, and cost effective. The RN provides initial and ongoing comprehensive assessment to include a review of systems which establishes a comprehensive plan of care. Triages and assesses all data received from HT patients such as vital signs, reported symptoms and question responses. The Care Coordinator will review all patient responses each work day and contact patients with high risk responses and trends, significant changes in condition or changes in other specific data elements received as clinically appropriate. Identifies and intervenes for potential exacerbations or complications to facilitate timely care in clinic, ER/urgent care, or care in the community. Provides appropriate interventions such as medication management, case management and patient education. Triages incoming calls and concerns of patients/families, resolve those within scope of practice and route others to interdisciplinary team staff or other services as indicated.
