Registered Nurse -Mental Health Intensive Case Management - Lubbock, TX - Overview Summary Duties The RN Case Manager position is accountable to the MHICM Program Director and Lead Nurse within Mental Health Service. Incumbent will collaborate with interdisciplinary team members in the MHICM program, facility and community. - Responsible for providing and evaluating care provided to the veterans in the community. Valid driving license and safe driving record required. - Completion of required clinical records as well as MHICM forms for evaluation and monitoring of service delivery and outcome. - Provides intensive case management with no less than one face to face visit per week or as determined through assessment with approval of Program Lead. Provides leadership in managing and coordinating care for Veteran's. Assumes responsibility and accountability for actions. - Assist the patient to develop knowledge and skills to improve and maintain his or her health and mental capabilities. - Completes initial assessments and reassessments of patients condition, response to treatment and other clinical findings in accordance to program policies and procedures. Initiates follow-up as needed and documents using clear concise and objective data. - Provides education and training to the veteran and/or family for identified educational needs and those to promote routine daily living functions. - Advise veteran on functions that are essential to personal safety, self-preservation and comfort in the community. - Systematically evaluate the current clinical needs of MHICM veterans, develop treatment plan and initiate action to promote optimal level of functioning. Investigates complex patient care problems and promptly seeks solutions/consultation and reports information within interdisciplinary team. - Performs crisis planning with Veteran and responds to crisis's and emergencies. Intervenes effectively and therapeutically with Veteran /family and utilizes de-escalation techniques as needed. Performs Suicide/Homicide Risk assessments and planning according to results.
