    FAPT (Family Assessment and Planning Team ) Coordinator Job ID 2018-3003 Position Type Full Time - Regular Hiring Pay/Range $25.79 - $27.79 Closing Date 04/22/2018 Department Human Services Reporting Location : Location US-CO-Fort Collins Overview Details The FAPT position is responsible for working in the Child Welfare Division of the Department of Human Services coordinating and managing the Family Assessment and Planning Team (FAPT). The purpose of the Family Assessment and Planning Team (FAPT) is to develop child centered, family focused, community-based services for children in out of home placement or at risk of out of home placement. The FAP Team focuses on providing a wrap-around service support system designed by the family utilizing clinical treatment services, natural supports and community supports. Responsibilities include arranging and facilitating family meetings, developing multi-systemic service plan, and connecting families with necessary resources. This position is responsible for data management, budgeting, and community outreach working with a variety of people from the families involved, probation, mental health and the two school districts. Specifically this person: Facilitates FAPT meetings and helps implement recommendations from those FAPT meetings. Coordinates and administers program services within established guidelines and policies and typically in compliance with program grant objectives. Provides direct client or public service and assistance; conducts training or educational programs. Evaluates program effectiveness, assess the needs of the community, and recommends improvements to program procedures and services to meet those needs. Provides coordination of student interns and/or volunteers within the department or program which may include scheduling, assigning and reviewing work, and instructing/training in work methods. Promotes public awareness and education on program services including developing and distributing brochures, developing outreach plans, and making presentations to community groups and collateral agencies. Coordinates program activities and services with other County departments and divisions, community groups, and collateral agencies; develops community resources. Acts as a liaison for assigned program to related programs and the public. Responds to complaints and requests for information and act as technical and referral source for clients and public regarding assigned and related programs and services. Participates in various committees and boards related to program activities and responsibilities. Interprets department and program policy and procedures. Compiles and maintains program records and statistics and prepare reports related to program activities and services; monitors program budget and grants; participates in budget development. Qualifications: At least two (2) years of experience as a professional social caseworker in either a public or private agency, including experience equivalent to a Social Caseworker II in Larimer County. Equivalent to a Bachelor's Degree from an accredited college or university with major coursework in a Human Services, Social Work, Sociology, Psychology, or a related Human Behavioral Sciences field. In order to meet the minimum educational requirements of a human behavioral science degree, the applicant must have a degree with major course work (equivalent to 30 semester hours or 45 quarter hours) in either development of human behavior, child development, family intervention techniques, diagnostic measures or therapeutic techniques such as social work, psychology, sociology, guidance and counseling, and child development. A Master's Degree in an appropriate field as defined above may substitute for experience. Successful Fundamentals of Child Welfare Case Practice completion and certification by the State Child Welfare Training academy. Maintain continuing education hours annually or as required. Possession of, or ability to obtain, a valid Driver's License. An offer of employment is contingent upon the successful completion of a pre-employment drug screen.
