    Maximus is currently hiring for a Lead Registered Nurse (RN) Evaluator to support our C-YES Program (Children & Youth Evaluation Service). The incumbent in this position is a subject matter expert on the CANS-NY regarding HCBS/LOC eligibility assessment for C-YES and serves as a role model, mentor, and resource for the Nurse Evaluators.

    About Children & Youth Evaluation Service

    The Children and Youth Evaluation Service (C-YES) is a New York State Program that determines Home and Community Based Service/Level of Care (HCBS/LOC) eligibility for youth with medical, behavioral, and/or developmental needs aged 0-21 years. The C-YES program also provides Medicaid application assistance services to children who appear HCBS eligible and are either without Medicaid and/or have Medicaid but opt out of HH Care Management. C-YES also develops person-centered Plans of Care (POCs) and provides ongoing HCBS service coordination for those families who elect to remain with C-YES and opt out of more comprehensive case management provided by Health Home Care Management Staff. HCBS are meant to help youths remain in their community as opposed to going to a higher level of care (i.e., facility or institution). A Care Coordination team comprised of Nurse Evaluators and Family Support Coordinators are responsible for ongoing support to all families referred to C-YES.

    **This is a hybrid remote position that will support Downstate New York. Travel will be required at times and will include home visits for assessment purposes.

    Essential Duties & Responsibilities:

    • Responsible for conducting clinical appointments and precepting new clinicians
    • Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments
    • Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes
    • Work collaboratively with manager to identify areas for improvement with process and quality of care
    • Suggest training needs for assigned unit staff
    • Provide technical assistance to assigned unit staff
    • Support Company and project initiatives through various methods including messaging to the unit, leading assigned projects, training staff, participating in work groups, etc.
    • Participate in performance, improvement, and quality assurance activities and related meetings
    • Provide clinical and educational support to meet program goals
    • Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position -Performs in-home and telehealth evaluations of patients that includes evaluation of pediatric consumers with a Serious Emotional Disturbance (SED) diagnosis or Medical Fragility (MF) diagnosis ages 0-21 years
    • Establishes and maintains positive relationships with consumers, caregivers, and all providers
    • Maintains updated knowledge of the UAS-NY CANS HCBS/LOC Assessment process.
    • Review’s consumer supporting documentation and/or referral forms, as relevant to the consumer case.
    • Enters evaluation data into the electronic HCBS/LOC Eligibility assessment.
    • Ensures completeness and accuracy of the HCBS/LOC Eligibility Assessment.
    • Documents any concerns, conflicting information, or other issues that surface during the evaluation process.
    • Emphasizes continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in Plan of Care.
    • Facilitates, mentors, and assists new C-YES Nurse Evaluators to meet the essential functions of the Nurse Evaluator role.
    • Conducts C-YES appointment visits with new Nurse Evaluators to provide demonstration and education. Instructs, demonstrates, observes assessment skills and techniques, and provides feedback to the Regional Nurse Manager.
    • Maintains and meets/exceeds set productivity standards.
    • Raises issues of concern and training needs to the Regional Nurse
    • Assists the Regional Nurse Manager in providing assessment guidance to team members
    • Assists with training of new Nurse Evaluators and refresher trainings as assigned for existing nurses.
    • Provides technical leadership to assigned staff.
    • Identifies areas in need of improvement and assists Nurse Evaluator and RNM in creating a plan to enhance skills.
    • Notifies Regional Nurse Manager of any delays or problems related to achieving an orientee's goals as soon as identified to ensure immediate intervention for optimal success.
    • Serves as a resource by guiding new team members to appropriate resource materials.
    • Nurtures, supports, and guides new and current team members through mentoring.
    • Meets all standards established for this position as outlined in the corresponding annual performance criteria for this position.
    • Performs other duties assigned by the Regional Nurse Manager.

    Minimum Requirements

    • Current Registered Nurse (RN) license valid in the state of practice is required
    • Two (2) or more years practicing experience as a Registered Nurse
    • At least one (1) year of experience in training, precepting, acting as a clinical advisor, providing unit support, working in the field independently/successfully
    • Must have active ID Number or be willing to obtain
    • Preferred work experience with the adult/geriatric populations; home care experience, experience working with individuals with physical or developmental disabilities- Required ability to travel throughout assigned region, up to 60% -Graduate of an accredited nursing program -Active New York State Driver’s License -Ability to travel within the state of New York, as assigned -Ability to review CANS HCBS/LOC questions and complete person-centered assessments and plans of care -Ability to work with technology remotely -Knowledge of the theory, principles, and practices of general professional nursing -Knowledge of state and agency laws and regulations governing general professional nursing practices -Excellent organizational, interpersonal, written, and verbal communication skills -Ability to communicate and work effectively with pediatric consumers 0-21 years of age -Ability to demonstrate sensitivity to the concerns of others -Ability to perform comfortably in a fast-paced, deadline-oriented work environment -Ability to successfully execute many complex tasks simultaneously -Possess a strong ability to work independently

    Preferred Requirements: -BSN in Nursing and/or equivalent certification -RN experience in a psychiatric / behavioral health / pediatrics / foster care / group home setting. -Home care experience using the UAS-CANS-NY system; -Children or Adult Home & Community-Based Services; -Case Management experience -Previous experience in precepting -6 months experience with HCBS/LOC assessment

    Pay Transparency

    Maximus compensation is based on various factors including but not limited to job location, a candidate's education, training, experience, expected quality and quantity of work, required travel (if any), external market and internal value analysis including seniority and merit systems, as well as internal pay alignment. Annual salary is just one component of Maximus's total compensation package. Other rewards may include short- and long-term incentives as well as program-specific awards. Compensation ranges may differ based on contract value but will be commensurate with job duties and relevant work experience.
