POSITION SUMMARY Provides community-bases services to individuals and
families by gathering information from clients, family and other
available resources, determining the individuals' specific problems, and
assisting in developing an action plan and/or mutual responsibility
agreement. This is the first level of professional social work.
ESSENTIAL FUNCTIONS Duties may vary depending on area of assignment. *
Assesses needs of individuals and/or families. * Interviews clients,
obtains family, social, and/or medical information. * Determines if
referrals are appropriate and whether clients are eligible for sponsored
programs. * Provides counseling and services in both individual and
group settings. * Discusses goals, expected outcomes and means with
which to meet goals and expected outcomes. * Assists clients in the
completion of required paperwork for items that may determine program
eligibility, referrals, or provide job assistance, such as job
applications. * Provides transportation services as required. * Manages
records and documentation of services provided and participates in team
case conferences. * Acts as a liaison with other staff with referrals
and transitioning of clients. * Coordinates benefits and referrals to
other agencies, whether internal or community based. * Transports
clients to appointments such as medical, job related or others, if
appropriate. * Ensures timely payments for ancillary services, if
appropriate and eligible. * Interview clients individually, in families,
or in groups, assessing their situations, capabilities, and problems, to
determine what services are required to meet their needs. * Serve as
liaisons between students, homes, schools, family services, child
guidance clinics, courts, protective services, doctors, and other
contacts, to help children who face problems such as disabilities,
abuse, or poverty. * Maintain case history records and prepare reports.
- Refer clients to community resources for services such as job
placement, debt counseling, legal aid, housing, medical treatment, or
financial assistance, and provide concrete information, such as where to
go and how to apply. * Develop and review service plans in consultation
with clients, and perform follow-ups assessing the quantity and quality
of services provided. * Provide, find, or arrange for support services,
such as child care, homemaker service, prenatal care, substance abuse
treatment, job training, counseling, or parenting classes, to prevent
more serious problems from developing. * Determine clients' eligibility
for financial assistance. * Serve on policymaking committees, assist in
community development, and assist client groups by lobbying for
solutions to problems. * May identify people who need help. * Assess
clients needs, situations, strengths, and support networks to determine
their goals. * Develop plans to improve their clients well-being. * Help
clients adjust to changes and challenges in their lives, such as
illness, divorce, or unemployment. * Research and refer clients to
community resources, such as food stamps, child care, and health care. *
Help clients work with government agencies to apply for and receive
benefits. * Advocate for and help clients get resources that would
improve their well-being. * Follow up with clients to ensure that their
situations have improved. * Evaluate services provided to ensure that
they are effective. MINIMUM QUALIFICATIONS Experience: Experience
requirements vary based on degree held by applicant. Education: MSW from
an accredited school of social work; BSW from an accredited school of
social work, Bachelors degree in a related field with 15 hours of social
work/counseling courses and one year of social work experience; or,
Master's degree in a related field. Equivalent Experience for Education
Accepted? N/A Licenses and Certifications: Must possess a valid NC or SC
drivers license. Must be able to obtain and maintain a County Operator
Permit. Knowledge of: * General social work principles and available
resources. * Social services procedures, practices and methods. * Social
services related federal, state and local laws, rules and regulations
relating to provision of public assistance, and social welfare. * Case
management methods and techniques. * Services offered by the county and
other local public and private community based service agencies. Skilled
in: * Possessing strong communication skills. * Managing multiple tasks
concurrently. Abilities: * Care Management - Having the knowledge and
skills to secure relevant patient care data and identify key issues and
relationships in the management of patient care; understanding the
relationships among assessments, interventions, and patient responses;
skilled in documentation of assessments and adjusting physical and
cognitive prescriptions as needed. * Work Standards - Setting high
