Care Management Coordinator -Position Type: -Department:Behavioral Health Division (Watertown Plank Location) -Compensation Range -Closing Date: -The Behavioral Health Division provides care and treatment to adults, children, and adolescents with mental illness, substance use disorders, and intellectual disabilities through both County-operated programs and contracts with community agencies. Services include intensive short-term treatment through our crisis services and inpatient services, as well as a full array of supportive community services for persons with serious mental illness and substance use disorders. Job Summary:Under the direction of the Care Coordinator Supervisor this position will coordinate care for the Rehab Central clients which have relocated into the community. In addition, the Care Management Coordinator will provide crisis stabilization for clients who are in need of short-term crisis care management. They will also serve as an initial contact for individuals who are new to the Behavioral Health Department (BHD). Job Responsibilities: -Conducts individual's assessments, provides service authorization, collaborates with referral sources, and develops individualized client care plans to promote recovery goals which are measurable under state guidelines. -Delivers Crisis Case Management services to individuals until formal services can be obtained or the consumer has stabilized. This will include documentation as outlined in state guidelines. -Creates Individualized Recovery Plans as needed and enters progress, maintain program records, reports, and legal documents. Follows the documentation guidelines outlined in the Medicaid Provider Handbook. Regularly enters information into the Electronic Medical Record (EMR) system. -Are you able and willing to travel (approximately 50% of the time) within the community to clients homes, schools and other settings to intervene in crisis situations. -Performs all other related duties as assigned. -Minimum Qualifications -Must be a current resident of the State of Wisconsin. -Current Wisconsin Driver's License and access to a reliable insured vehicle; both maintained throughout employment. -Bachelors Degree or higher in Human Services, Psychology, Social Work or Occupational Therapy is required. -If you have a degree in Occupational Therapy or Social Work you must also be licensed or certified by the state of Wisconsin. -At least (3) years of related work experience in a Casework or Care Management role is required. -At least an intermediate knowledge of Microsoft Office software (i.e. Microsoft Word and Excel). -Ability to travel (approximately 50% of the time) within the community to clients homes, schools and other settings to intervene in crisis situations?
