    Job Description

    Under the supervision of the WPIC Director of Nursing, the Nurse Manager supervises the delivery and evaluation of nursing care of medically and psycho-socially vulnerable clients who are experiencing homelessness, staying in shelters or living in supportive housing sites. The Nurse Manager oversees the teams responsible for assessment, care plan development and coordination of care for some of the city’s most complex clients. The program goal is to stabilize clients wherever they are living, prevent hospital admission and advocate for/refer clients to the appropriate level of care. The Nurse Manager coordinates all nursing support services to ensure the health and safety of clients. The Nurse Manager will work closely with other WPIC program nurse managers.

    The Nurse Manager performs the following essential job functions:

    • Assesses, plans, implements, and evaluates care using patient outcomes data
    • Collaborates managing common health care problems present in homeless populations and develops innovative interventions and models to improve care of high risk/high vulnerability individuals experiencing homelessness.
    • Leads or participates in WPIC clinical case conferences, other case conferences, meetings, nursing staff meetings and administrative / management meetings
    • Participates in quality improvement efforts such as conducting quality control (QC) activities, evaluating critical incidents/unusual occurrences/patient safety issues, and overseeing an overall quality assurance program
    • Coordinates hiring, provides supervision, coordinates assignments/scheduling, and completes performance appraisals for a multi-level and multi-cultural staff of registered nurses, licensed vocational nurses, health workers and public health aides
    • Develops, maintains, and updates nursing and infection control and hazardous waste policies, procedures, and standards to maintain compliance
    • Develops and maintains emergency response plan and coordinates dissemination of plan to program staff
    • Ensures that all occupational injuries and illnesses are reported per departmental protocol
    • Maintains occupational health program for the screening of clinic employees for select communicable diseases and compliance of Departmental and Division program
    • Maintains systems for ordering and inventory as well as for receiving and responding to patient complaints
    • Participates and collaborates with members of the administrative team to develop systems that facilitate safe, efficient, comprehensive and client-centered care
    • Serves as liaison between RESTORE program and nursing leadership within SFDPH / SFHN Ambulatory Care

    The 2322 Nurse Manager also performs other related duties as required.

    Minimum Qualifications

    1. Possession of a valid California Registered Nurse License; AND
    2. Possession of a baccalaureate degree from an accredited college and/or university in Nursing, Nursing Administration, or a closely related field; AND
    3. Whole Person Integrated Care (WPIC) Specialty: Two (2) years (equivalent to 4,000 hours) of Registered Nurse experience working with individuals experiencing homelessness, substance use disorder, or co-existing psychiatric conditions.

    Substitution

    • Up to two years of additional experience as a Registered Nurse working with individuals experiencing homelessness, substance use disorder, or co-existing psychiatric conditions may be substituted for the required degree on a year-for-year basis. Thirty (30) semester or forty-five (45) quarter units equal one year.-

    Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.

    One-year full-time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40-hour work week).

    DESIRABLE QUALIFICATIONS: The stated desirable qualifications may be considered at the end of the selection process when candidates are referred for hiring.

    • Clinical experience in Homeless Healthcare
    • Knowledge of MOUD/MAT for people with opioid use disorder
    • Experience in supervising a large multi-disciplinary clinical team/staff
    • Sensitivity to a multi-cultural, multi-diagnosed, diverse, urban patient population
    • Ability to work well with diverse staff, collaborators, and patients including cultural, language, sexual identity, gender and other diversity considerations
    • Has prior training and/or direct work or community service experience in anti-racism, racial justice, and/or racial equity initiatives and/or projects.
    • Sensitivity to and experience working with LGBTQ and gender non-confirming individuals
    • Individuals who have lived experience with Black/African American populations
    • Experience in supervising nursing staff and developing and enforcing operational procedures
    • Strong clinical skills
    • Ability to work as part of a team, prioritize and handle multiple tasks and to work independently in a high-pressure environment
    • Excellent oral and written communication skills
    • Excellent problem-solving skills.
    • High level of accountability, with the ability to work independently
    • Excellent attendance record
    • Previous experience or training in harm reduction
    • Previous experience or training in street outreach and healthcare for homeless people
    • Experience with patient data collection, patient documentation and healthcare quality management
    • Knowledge of management theories/principles; systems and change theories; conflict resolution and customer service skills
    • Ability to conduct chart audits and investigations, critically analyze adverse outcomes and ascertain/write action plans

    Selection Procedures:

The MQSQ will be used to evaluate if the applicant possesses the required minimum qualifications.

    Training and Experience Exam: (Weight: 100%)

    The Training and Experience Exam is designed to measure the knowledge, skills, and abilities in job related areas which may include but not be limited to:

    • Knowledge of nursing theory and practice; Ability to plan, organize, coordinate and evaluate nursing activities, Knowledge of problem solving process, Ability to supervise, Ability to communicate effectively

    Candidates must achieve a passing score on the Training and Experience exam to continue in the selection process and will be placed on the confidential eligible list in rank order according to their final score.

    Additional selection processes may be conducted by the hiring department prior to making final hiring decisions.

    Certification

    The certification rule for the eligible list resulting from this examination will be the Rule of Ten Scores.

    Eligible List/Score Report:

    A confidential eligible list of applicant names that have passed the civil service examination process will be created and used for certification purposes only. An examination score report will be established, so applicants can view the ranks, final scores and number of eligible candidates. Applicant information, including names of applicants on the eligible list, shall not be made public unless required by law. However, an eligible list shall be made available for public inspection, upon request, once the eligible list is exhausted or expired and referrals resolved. The eligible list/score report resulting from this civil service examination process is subject to change after adoption (e.g., as a result of appeals), as directed by the Human Resources Director or the Civil Service Commission.

    The duration of the eligible list resulting from this examination process will be of six months and may be extended with the approval of the Human Resources Director.
