    Your role at PFH:

As a Therapist, you are responsible for assisting individuals with mental illness and/or substance use disorders by providing the necessary support and guidance to help clients improve their overall well-being. You will:

    • Complete Intake/Screening and Assessment: Conduct thorough intake interviews to gather information about the client's history, presenting concerns, and goals for treatment. This initial assessment helps you understand the client's needs and tailor your approach accordingly.
    • Diagnosis and Documentation: Use your clinical expertise to determine an accurate diagnosis that aligns with the client's symptoms and experiences. Documenting the diagnosis and assessment findings is essential for treatment planning and ensuring continuity of care.
    • Facilitate Various Therapy Sessions: You'll conduct various types of therapy sessions, such as group counseling, family therapy, and individual sessions. These sessions provide safe spaces for clients to explore their concerns, learn coping skills, and receive support.
    • Develop Individualized Treatment Plans: Crafting treatment plans that adhere to state regulatory or accreditation guidelines is crucial while ensuring each plan is individualized, considering the client's unique strengths, challenges, and goals.
    • Handle Crisis Situations: You will be trained in crisis intervention techniques to manage these situations effectively. Your role may involve assessing risk, providing immediate support, and collaborating with other professionals if necessary.
    • Assist Clients Through Transition: As clients progress in their treatment, they may transition to different levels of care or external services. You'll guide them through this process, ensuring a smooth transition and providing any necessary information or referrals. Completing final correspondence like discharge summaries or plans helps maintain a clear record of the client's progress.
