    YOUR ROLE AT PFH:

    As a Family Advocate, you are responsible for responsible for providing direct outreach, child and family team coordination, home, school and community visitation, development of individualized Treatment Plans, coordination and delivery of individualized services across all settings, documentation of team related activities and services, and advocacy services to high-risk families. You will:

    • Represent PFH and clients served in a positive manner to clients, staff, community agencies, and community members.
    • Advocate for clients.
    • Manage assigned caseload of clients and their families, depending on severity level.
    • Participate in wraparound process, facilitate Child and Family Team meetings and services, create and monitor individualized Treatment Plan, ongoing delivery and implementation of services as indicated in each treatment plan.
    • Become completely familiar with each client's current Treatment Plan, initiate teaching, treatment and supporting interventions to assist clients in obtaining their goals. Monitor and document progress.
    • Develop a positive, non-judgmental relationship with each child and family member, modeling appropriate behavior while acting as a role model for them.
    • Administer assessment instruments as assigned.
    • Teach daily living skills as needed related to budgeting, home cleanliness and organization, diet and menu planning, shopping, healthy and nutritional eating habits through teaching, modeling and monitoring.
    • Planning and implementation of recreational, educational, and vocational act.

    WE THINK YOU'D BE GREAT FOR THIS ROLE IF YOU HAVE:

    • Bachelor's Degree from an accredited college or university in the field of psychology, social work, sociology, education, nursing or other human service related field. An individual with a bachelor's degree in a human services field, which includes social work, psychology, nursing, education, criminal justice, recreational therapy, human development and family studies, counseling, child development, gerontology, sociology, human services, behavioral science and rehabilitation counseling;
    • Applicants without a bachelor's degree may be considered with a combination of PFH approved education and experience.
    • Must have and maintain certification in CPR, First Aid and Blood borne Pathogens.
    • Must have a valid driver's license, acceptable driving record and current insurance.
    • Acquire and maintain Substance Abuse Credentials at the highest level allowed by the Missouri Credentialing Board.
