    Your role at PFH: 

    As a Registered Nurse, you are responsible for monitoring the safety and well-being of persons served in the residential services program You will:

    • Conduct monthly face to face visits of each residential client in their home
    • Complete an assessment/evaluation at the time of the visit and document findings on the Residential RN Monthly Health Summary.  This includes:
      • Review, analyze and interpret lab results.  Ensure there is a system in place for lab work and follow-up
      • Review of the physician's orders for physician signature, accuracy (including diagnosis) and staff compliance with the orders;
      • Review any new physician's orders and document the review with a signature and date.
      • Review of the MAR (medication administration record) for accuracy (compare to physician's orders) and staff compliance with administration, documentation, and follow-up of PRN medications; 
      • Review of Comprehensive Health Notations;
      • Review of special diets, blood sugar, blood pressure, and choking risk protocols and ensure these are being followed and documented by staff;
    • Ensure a copy of the completed assessment is in the client file in the home and a copy to the Program Coordinator/Manager.
    • Maintain communication with agency management and the client's support team including reporting and documenting all changes in client's health status and needs.
    • Assist RD in completing Nursing Action Plans in CIMOR.
    • Ensuring needs for staff supervision/delegation or specialized instruction are conducted and documented on the designation form (Delegation of Specialized Nursing Tasks). Recording initial patient medical histories and vital signs
