YOUR ROLE AT PFH:
As a Community Health Worker, you play a vital role in improving community health by providing a connection between health providers and community resources as well as educating patients on how to reduce health risk factors.'
You will:
- Facilitate communication and patient empowerment in interactions with healthcare/social service systems.
- Troubleshoots any barriers the patient might have in adhering to care.
- Engage in the community through outreach to recruit and engage high-risk patients.
- Serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery.
- Assist the primary care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease.
WE THINK YOU'D BE GREAT FOR THIS ROLE IF YOU HAVE:
- Undergraduate Bachelor's degree in Social Work, Psychology, Public Health Nursing, Health Education, or a related field from an accredited school.
- Two (2) years of experience working in community health, human services outreach and/or healthcare preferred.
- Desirable experience includes: customer service, patient/peer coaching, medical patient interaction, community relations, outreach, data entry and public speaking.
