    YOUR ROLE AT PFH:

    As a Family Coordinator, you are responsible promoting coordination and integration of family-centered client care to ensure participation and involvement of the entire family.

    You will:

    • Respond to crises by facilitating immediate connections to services for all referred potential program participants including immediate connection at the time and location, if required, of referral to services and resources coordinated through strong advocacy and case management with all appropriate parties involved;
    • Promote service integration and community resource connections with project participants;
    • Provide family-centered care, encourage family involvement, family-focused interventions;
    • Develop treatment plans for clients in accordance with DMH/CARF guidelines that demonstrate evolution and individualization based on information gathered from assessment
    • Employ case management strategies to address SUD/COD and primary health conditions and link with and retain project participants in necessary services;
    • Provide requested documentation to Project Coordinator and PFH Grant writing team in regards to outcome measures, to help facilitate the completion of reports required by funding sources;

    WE THINK YOU'D BE GREAT FOR THIS ROLE IF YOU HAVE:

    • Graduate from an accredited college or university with a Master's Degree in social work, counseling, public health, education, psychology, community development, public administration or a closely related field, who has at least one (1) year of professional experience in a social service agency;
    • Certification from the Missouri Credentialing Board, as a MAADC II or higher.

    Applicant must be willing to become licensed in additional states, other than the state currently licensed in. Applicant must be willing and comfortable with providing telehealth services to the individuals we serve.
