SUPPORT COORDINATOR Division of Developmental Disabilities 400 West Congress Tucson, Arizona 85701 www.azdes.gov JOB SUMMARY: The Case manager 2 serves members with developmental disabilities, e.g., Autism, Cerebral Palsy, Intellectual Disability, Epilepsy, etc., by supporting them and their families to lead the healthiest, most self-sufficient, productive lives within their capabilities. Incumbent manages an ongoing caseload, and gathers all necessary information and documentation for determining eligibility and/re-determining eligibility for DDD Services following all relevant state, federal mandates, agency policy and procedures relating to Early Intervention, education programs, behavioral health, vocational rehabilitation services, AZ Health Care Cost Containment System (AHCCCS) Long-term Care, Targeted Support Coordination and state contracted/non-contracted agencies. JOB DUTIES: * In compliance with requirements, drive vehicle to conduct on-site visits to individuals/families, homes, schools and program sites to develop relationships and family dynamics. * Discuss service needs and support services. * Develop and write the Individual Service Plan (ISP). * Document individual progress and concerns. * Complete appropriate referrals to community agencies. * Research community for available resources to meet needs identified in (ISP) and to inform individual of resource options. * Facilitate meetings, program visits and attend meetings to assist with individual/family choices. * Complete referrals for assessed/covered services. * Develop, review, monitor and evaluate (ISP) in accordance with mandated timelines. * Approve type and level of assessed services within approved guidelines. * Collaborate with community agencies to provide assistance and support to individuals and families in crisis. * Write comprehensive progress notes and maintain case records. * Prepare a variety of reports and complete special projects, as required. KNOWLEDGE, SKILLS AND ABILITIES (KSAs): Knowledge of: * Methods procedures and techniques used in the development of Individual Support Plans (ISP) * Federal, state statues, agency policies and procedures relating to the care and assistance for individuals with developmental disabilities * Principle and practices of case management

    • Community resources * Medical and behavioral health issues and terminology * Fundamentals of applied behavioral interventions * Child development regulations Skill in: * Oral and written community * Developing, evaluating and maintaining member service plans * Interviewing, counseling and interacting with members and family members
    • Conducting needs assessments * Problem identification and resolution * Interpersonal relations as applied to contacts with members and representatives of other governmental and community organizations and the public * Conflict resolution * The use of various software (Microsoft Word, Excel, etc.) Ability to: * Work Independently and within diverse work teams * Elicit accurate data from a variety of sources * Absorb, interpret and assimilate large quantities of written material * Organize and manage case load * Document case files and information within required/mandated timelines * Maintain confidentiality * Drive a motor vehicle SELECTIVE PREFERENCE(S): * The candidate will possess two (2) years of professional experience coordinating social service activities (e.g., ISP(s) client referral for services, community services, etc.); Or one (1) year as a Human Services Specialist I in Arizona State Services; Or a Bachelors or Masters degree from an accredited college or university * In-state travel is required * Depending on the profile and the members served there may be a bilingual selective preference PRE-EMPLOYMENT REQUIREMENTS: * Have a current, valid state issued drivers license appropriate to the assignment. Employee is subject to drivers license record checks and must maintain an acceptable driving record, valid motor vehicle insurance and registration, and must complete any required training (see Arizona Administrative Code R2-10-207.12) * Successfully pass background and reference checks.
