    Community Living Coach United Cerebral Palsy of GA 9 reviews - Stone Mountain, GA Part-time JOB TITLE: Community Living Coach (CLC) DIVISION: Community Residential Alternatives (CRA) SUPERVISOR: Home Manager FLSA STATUS: Non-Exempt JOB SUMMARY: Training and care of residents with multiple disabilities, including all areas of daily life. EDUCATION OR TRAINING REQUIREMENT: High School diploma or equivalent. EXPERIENCE REQUIREMENT: 1-2 years experience working with individuals with disabilities and/or related field. Must have basic computer knowledge and proficiency. SPECIAL LICENSES, REGISTRATION OR CERTIFICATION: First Aid, CPR, and Georgia or South Carolina Drivers License SUPERVISORY RESPONSIBILITY: None ESSENTIAL JOB DUTIES AND RESPONSIBILITIES: Training/Program Responsibilities A. Implements activities suggested by supervisor which reflect training objectives in each clients individual service plan. B. Engages residents in activities of choice, which reinforces ISP goals. C. Promptly reports any emergency/crisis situation to supervisor and follows up by completing appropriate incident report within twenty-four (24) hours. D. Provides transportation and supervision for residents as dictated by individual program. E. Ensures that all information regarding daily activities, problems and needs of residents are recorded properly in daily log. F. Ensures each residents personal hygiene and grooming needs are met. G. Provides training and assistance in changing appropriate apparel on a daily basis to ensure comfort and well being for residents. H. Assists residents and/or pack lunch preparation as needed. I. Uses appropriate body mechanics to lift, move, and position and provides prescribed exercises and physical care to residents. II. Responsibilities Relating to Health and Safety A. Inspects home at beginning of shift to ensure that home meets all safety requirements, checking for clutter and dangerous objects. B. Corrects deficiencies or submit written report to supervisor outlining deficiencies by end of shift. C. Conducts evacuation drills, both fire and severe weather, as directed by supervisor. Records results of each drill. D. Follow program plan for meeting all potential emergencies and disasters, such as medical, fire, severe weather and missing persons. Discusses with supervisor specific role relating to each emergency. E. Notifies supervisor of all medical emergencies as outlined by policies and procedures. F. Checks to ensure that all flammable materials are properly stored and safeguarded. G. Checks to ensure that supplies are clearly labeled. H. Checks to ensure that food is protected from contamination and spoilage. Checks freezer and refrigerator to ensure that food is properly wrapped and/or covered. Corrects any problem and reports to supervisor for further corrective action. I. Cleans and sanitizes all appliances, utensils and work surfaces in accordance with the Personal Care Home Rules and Regulations guidelines. J. Checks cooking surfaces exhaust hoods and oven. Properly washes/cleans and maintains in a safe manner. K. Disposes of garbage daily sanitizing containers and lids. L. Checks plastic ware, china, glassware or other similar items for ships or cracks. Places in a box to be picked up and removed from inventory in accordance with policy/procedure of UCP Residential Services. M. Reports any problem with equipment used by residents to supervisor. N. Cleans and sanitizes bathrooms, kitchen, and telephones daily in accordance with Personal Care Home guidelines. III. Housekeeping Responsibilities A. Checks home and performs all daily household cleaning chores to include, but not limited to, inside windows, window sashes, blinds, mirrors, carpets, etc. B. Ensures that laundry is washed, ironed and properly put away daily. C. Other housekeeping responsibilities as assigned by supervisor. IV. Administrative Responsibilities A. Ensures compliance in the area of responsibility during assigned hours with local, state and federal rules and regulations applying to Personal Care Home and UCP Residential Services. B. Complies with the organization policies and procedures governing the use of behavior management programs for controlling maladaptive or problem behavior. C. Complies with policies and procedures to ensure that confidentiality requirements are upheld. D. Supports and protects the fundamental human, civil, constitutional and statutory rights of clients and families as defined by agency. E. Demonstrates competence in handling medical emergencies in accordance with UCP policies and procedures. F. Maintains accurate and complete records of all residents personal spending monies in accordance with rules and regulations. G. Reports to work promptly and on time and does not leave work until assigned shift ends or has approval from supervisor. Observes policies/procedures governing requests for annual, sick or other leave time. H. Observes all other personnel policies and works closely with supervisor to ensure compliance. I. Abides by code of ethics and serves as a good role model for clients. V. Record Keeping Responsibilities A. Maintains records in the home to meet local, state and federal guidelines B. Makes client related entries in daily logs that are legible, dated, authenticated by signature and positions, in ink and without use of symbols or abbreviations. C. Maintains tracking records in accordance with Individual Service Plan to comply with program requirements and expectations of supervisor. D. Records contacts with clients family or guardian. E. Maintains current log for approved family visitations and/or outing with friends. F. Maintains medication records in accordance with policy G. Maintains records of evacuation drills H. Maintains appropriate documentation relating to health, safety and infection control. VI. Physical Health Service Responsibilities A. Review medical history of each resident. B. Receives training on supervising the self-administration of medications and follows all policies and procedures. C. Assumes responsibility for ensuring that all medications are kept under lock and key. Supervises self-administration of medications in accordance with established policies of Georgia Department of Human Resources by: a. Right resident b. Right medications c. Right dosage d. Right time e. Right route f. Right documentation D. Obtains annual TB test and ensures that results are signed by appropriate medical staff and on file in Center Personnel record. E. Considers the need and advisability of obtaining Hepatitis B Vaccine in accordance with UCP recommendation. F. Provides required documents for personnel file as requested, to include Georgia Drivers License, First Aid/CPR certification, physical, etc. G. Adheres to all agency policies and procedures and Personal Homes Care Rules and Regulations. H. Other duties as assigned. VII. Develop and maintain positive and productive relationship with consumers, families, co-workers, and community members. VIII. Maintain objectivity in position in order to set appropriate limits while working with consumers. IX.
