The American Heart Association has a Part-Time Hourly Community Impact Manager position (10 to 15 hours per week) available in the Western Tidewater (Hampton Roads) area. This position is grant-funded through August 31st, 2025. This will be a Hybrid position: 75% work from home, 25% community based. Please note that this program will be serving the Western Tidewater area, which includes cities of Franklin and Suffolk, and Isle of Wight, Southampton, Surry, and Sussex counties.
The Community Impact Manager is responsible for handling the clinical and operational metrics related to the No Pressure: A Clincial Blood Pressure Program initiative across Western Tidewater, working to improve hypertension control rates. Oversee practice assessment process for up to 10 participating Health Care Organizations (HCOs). Provide outstanding support to HCOs through relationship management in collaboration with local staff and leadership. Responsible for daily operations related to the grant measurables with strong attention to detail. Organize, plan, and complete supportive activities including, but not limited to, regular communications to cohort, sharing of resources, hosting bi-annual cohort meetings, and other CME and non-CME credit bearing events. Focusing on hypertension and clinical operations improvement and implementing policy of systems changes. Engage cohort HCOs to ensure best practices and quality improvement for treatment of patients through the American Heart Association’s outpatient programs: Target: BP, Check. Change. Control. Cholesterol, and Target: Type 2 Diabetes.
