Conducts Community Health Needs Assessment and Implementation plans for selected neighborhoods. Leads and supports health improvement projects and initiatives identified in Implementation plans. Responsibilities: 1. Lead role in conducting Community Health Needs Assessment process for a selected neighborhood Analyzes data and identifies opportunities for strategic health improvement efforts. Shares insights and information across organization to facilitate strategic thinking, innovation and link to neighborhoods. 2. Assists in development of Community Health Implementation Plan strategies and tactics for selected neighborhoods. Works in partnership with internal and external stakeholders to develop and implement community health improvement strategies based on in-depth knowledge of evidence-based practice, previous successful efforts, community partners and local community assets and resources. Also accountable for measurement of initiative impact. 3 Serves as a P-CHE Ambassador to the Community. Identifies and builds relationships with critical internal and external stakeholders. Attends significant community events representing P-CHE. 4. Fosters effective team interactions through open and honest communication, receptive to feedback, mentoring and developing others. Develops and supports relationships that create an environment where individuals and team members learn and grow professionally. 5. Participates in process improvement and problem-solving opportunities to enhance P-CHEs efficiency and effectiveness. 6. Evaluate programmatic feasibility and effectiveness 7. Tracks community benefit related to P-CHE and reports to Jefferson Hospitals as appropriate Education Required: Masters Degree in Public Health, Community Health, Health Education, Community Organizing, Health Sciences Preferred: Doctoral Degree in Public Health, Community Health, Health Education, Community Organizing, Health Sciences Required: 5 years of experience in public health, health education, community health, community organizing, urban planning Preferred: 10+ years of experience in public health, health education, community health, community organizing Additional Skills 1. Understands Community Health Improvement framework and processes 2. Understands Community Health Assessment and Prioritization processes 3. Ability to implement Community Health action plans 4. Skilled at leading and facilitating groups of people to achieve desired goals 5. Builds relationships and fosters collaboration with critical and diverse community partners 6.
