SERVICE COORDINATOR
Accra is a non-profit organization offering the best in personalized, self-directed home care. Accra provides employee, client, and billing services for various programs including Personal Care Assistance (PCA) Choice, Fiscal Management Services (FMS), and Home Health Agencies. We are located in Hopkins and offer services in all of Minnesota’s 87 counties, with offices in Minnetonka, Alexandria, Red Wing, Fergus Falls, Moorhead, Winona, Lake City, Duluth, Grand Rapids, Bemidji, Brainerd, and Virginia.
DUTIES:
- Meet with clients and their representatives to complete the intake process, review consumer support plans (CSPs) from the county and initiate services with Accra Consumer Choice.
- Monitor and document service delivery on an on-going basis.
- Manage caseload of clients with high level of customer service and mathematical skills.
- Educate clients on role and responsibilities as a participant-employer.
- Provide technical assistance and information to families.
- Communicate with the participant, representative, county case manager, referral sources, co-workers and Accra support services.
- Be comfortable in working with a variety of settings and communities.
- Coordinate service delivery and accommodation with other members of the Accra team, including Qualified Professionals, HR staff and others.
- Maintain knowledge and understanding of DHS HCBS waiver regulations, IRS, DOL, unemployment, worker’s compensation, liability, and best practices regarding participant-directed services.
- The ability to explain sometimes difficult concepts verbally and in writing to customers and coworkers.
- Ability to maintain a dependable work attendance and engage as part of a collaborative and integrated team.
QUALIFICATIONS:
- 4-year degree required with at least 1-year experience working with individuals with developmental and/or intellectual disabilities.
- Experience working with Waivered Services and/or PCA is a plus
- Ability to organize and prioritize
- Ability to handle numerous tasks simultaneously while maintaining strong attention to detail
- Maintain a high standard of professional courtesy, flexibility, confidentiality while working in a fast-paced office environment, independently and as a team member.
- Advanced skills in the use of office technology, including MS Office Suite with advanced skills in Word Excel and Outlook
- Valid driver’s license and must pass a background check
