Job Description
PROGRAM AIDE (9:00 AM -3:00PM)
OVERALL OBJECTIVE: Provides supportive services to the participant’s under the direction and supervision of a professional staff member, as assigned by the Program Director.
RESPONSIBLE TO: Activity Director and Nursing Department as assigned
RESPONSIBLE FOR: Direct participant care
AREAS OF RESPONSIBILITY:
- Assists other staff in carrying out program assignments.
- Works in conjunction with other staff in relation to participant care, as assigned by the Activity Director and the nursing department.
- Is responsible for order and cleanliness of the program areas and equipment.
- Responsible for providing maintenance program treatments as prescribed by the PT and/or OT under the supervision of the RN. Services shall only be provided these receiving the proper training from the PT and/or OT.
- Carry out assigned task as deemed necessary by the Activity Director and/or the RN.
- Provides personal care, such a toileting, bathing, eating, transferring, dressing or grooming under the supervision of the nurse.
- Assist the Activities Director in carrying out the activity program.
- Assist other center staff in meal and snack serving.
- Write notes and documents all services provided on flow sheets on the same day treatment is provided.
- Makes out own time card.
- Attends and actively participates in the assessment process by relating participation history.
- Attends center staff meetings and in-service educational training programs.

MINIMUM QUALIFICATIONS:
High school graduate or equivalent; The ability to comprehend both written and spoken English.
An interest and desire to work with the target population served by the center; Prefer bilingual (Spanish, etc.)
CONDITIONS OF EMPLOYMENT:
COVID-19 vaccinated, T.B. and physical clearance for reaching, bending, stooping, crouching, kneeling and grasping; have full range of motion; ability to lift 50 lbs; may need to lift and transfer participants; must have current First Aid and CPR; must have reliable private vehicle, proof of CA drivers license and provide proof of current vehicle insurance.

Active Life ADHC is a licensed and Medi-Cal certified CBAS Program (Community Based Adult Services) center located in San Joaquin County. We provide medical and social services in a daytime setting for adults ages 18 and up, including the frail elderly.
