  • Responsibilities

     

     

    The incumbent in this position will participate in the delivery of the KIC Tribal Health Clinic’s Diabetes Program and the Diabetes Prevention Programs and other health promotion programs and activities.  The position is housed in the Wellness Department and will provide medical nutrition therapy for patients with diabetes, other chronic diagnosis and nutrition related conditions. The person will also provide health education to individuals and groups with chronic care needs to assure best practices positive outcomes.  Other responsibilities include participation in a multi-disciplinary team of health care professionals from a variety of settings within KIC and Alaska Tribal Healthcare agencies. The activities and responsibilities of the position are carried out in accordance with grant guidelines, policies and procedures of KIC Tribal Health Clinic, Indian Health Service and American Diabetes Association standards of care.

     

     

    Required Skills

     

     

     

    • Provide medical nutrition therapy, develop and implement nutrition care plans for individuals and groups in outpatient and group settings;
    • Participate in health education for diabetes and chronic disease prevention projects and activities;
    • Assist with the monitoring and reporting to satisfy grant requirements and ensure their accuracy and timeliness in submission to supervisor, tribal government, IHS, AADE;
    • Monitors the overall quality of activities and projects; identify problem areas and communicate quality control, training, and other corrective measures as needed to supervisor and in accordance with grant and tribal guidelines;
    • Works collaboratively within the wellness team to maintain and develop processes necessary to provide high quality health promotion prevention and treatment education, classes, programs and activities;
    • Serves as a team member liaison by providing communication and collaboration with other health care agencies;
    • Assists the Wellness Manager in development, implementation, and evaluation of monitoring and tracking systems to assure continuity of care according to KIC, AAAHC, State, Federal and Indian Health Service standards of care;
    • Serves as Community Registered Dietitian Nutritionist resource to the staff regarding Wellness Department health promotion activities;
    • Conducts interdisciplinary education on the services and advantages of evidenced based best practices for Health Promotion and Nutrition processes, programs and activities including orientation of new employees to the Wellness/ Diabetes Case Management services;
    • Assists Wellness Manager with development of policies and procedures for clinic practices related to Wellness Department and other needs to assure compliance with accreditation standards.

    • Knowledge of screening and identification of symptoms of alcohol/drug abuse and withdrawal;
    • Knowledge and ability of Case Management skills;
    • Knowledge and ability to carry out rules of Health Insurance Portability and Accountability Act (HIPPAA), which protects patient rights;
    • Knowledge and ability to carry out federal confidentiality rules for clients seeking substance abuse services;
    • Skilled in the coordination of patient care in remote locations (phone consultations, etc.);
    • Ability to communicate effectively and professionally over the telephone and in person in a clear and positive manner;
    • Ability to provide social treatment services in a clinical and community-based setting;
    • Ability to exercise mature judgment in evaluating specific situations and recommending solutions;
    • Ability to listen, observes, and records a variety of information and situations;
    • Ability to collect data, keep accurate and organized records and ensure timely preparation of reports;
    • Ability to establish and maintain effective working relations with clients, peers, public and private agencies, Tribal members, and the general public;
    • Ability to utilize maximum degree of professionalism and diplomacy with those who may be experiencing high degrees of stress both mentally and/or physically;
    • Ability to be flexible in order to perform at the highest level as a team player;
    • Demonstrated expertise in: project management; Medicaid-compliant substance and mental health treatment; ability to plan and implement projects;
    • Demonstrate the ability to work independently with minimal supervision and able to prioritize assignments.

    Physical Activities Include

    Must be able to hear.  Specific vision abilities include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.  Ability to use hands to finger, handle, or operate objects tools, or controls; reach with hands and arms; climb or balance; stoop, kneel, crouch; and taste or smell.  Each of the listed activities also requires standing, sitting, walking, bending, and stooping, climbing on stools and ladders, and routinely lifting and carrying items and boxes up to forty (40) pounds.

    The work is mostly sedentary with some walking, climbing of stairs, carrying of files, or other documents and light supplies, bending and stooping to perform filing.

    Required Experience

     

    • Registered Dietitian Nutritionist (RDN) accredited by Commission on Dietetic Registration. 
    • Proficient in providing culturally appropriate 1:1 or small group counseling and patient education.
    • Certified Diabetes Educator (CDE) or eligible to apply for certification within two years of hire.
