  • Responsibilities

     

    Under limited supervision, coordinates the Maniilaq Diabetes Program’s Special Diabetes Program for Indians (SDPI) initiatives for diabetes prevention and community education. This position works to identify, educate, and build a solid program foundation to support our important diabetes prevention initiatives. They will also work in collaboration with existing programs to elevate improved outcomes.  Reports to the Diabetes Program Manager.

    PRINCIPAL DUTIES AND RESPONSIBILITIES

     

    Coordinates services for diabetes outreach branch of the Maniilaq Diabetes Program Special Diabetes Program for Indians grant, accomplishing program development, implementation, maintenance and evaluation of all outreach initiatives of the Diabetes Program.

    Collaborates with other programs and organizations to deliver to coordinate health promotion events, activities and dissemination of educational resource materials. Partners with other programs and organizations to deliver holistic, culturally honoring wellness programming and to promote wellness and diabetes prevention through effective self-management.

    Disseminates best practice guidelines to the Maniilaq Diabetes Program Staff via electronic means and uploads into the shared folder under Lifestyle Coaching Tool-kit. Maintains a quarterly calendar of prevention initiatives and utilizes technology and social media platforms to disseminate information about events on a timely manner to inform the public about activities happening in in the region.

    Provides feedback to the interdisciplinary team regarding current and developing policies and impact on health of clients, collects and disseminates relevant outcome data, incorporates standards of care, and participates in evaluation process.

    Provides Lifestyle Coaching including comprehensive assessment and appropriate intervention in collaboration with the interdisciplinary care team.

    • Effective utilizes the electronic health record to document encounters and communicate with the referral provider and the interdisciplinary team.
    • Timely receives Lifestyle Coaching referrals through the electronic earth record via Message Center
    • Monitors appoints booked in the scheduler book. May need to schedule appointments when administrative assistant not available.
    • Provide individual and groups/classes patient education for the communities in the Maniilaq serving region and community stakeholders.
    • Provides extensive information on diabetes prevention to patients, staff, and community groups as requested.

    Periodically reviews the health status and needs of Native Americans by involving patients and families through surveys and/or focus groups, to identify health problems and develop educational opportunities, activities, and utilization of facilities.

    Utilizes the registry (diabetes, pre-diabetes, GDM, etc.) to provide recommendations for population health strategies related to diabetes outcomes.

    Plan and execute the Diabetes Prevention Program (DPP) classes and other initiatives (e.g. Diabetes Prevention Youth Camps, Boy’s and Girl’s club monthly activities, social media and PSAs).

    • Review, evaluate, and monitor Diabetes Program Wellness and Prevention initiatives to ensure adherence to funding objectives including but not limited to outreach, registration, session implementation, curriculum development, wellness activities, data collection, patient education, while ensuring participation tracking and reporting of outcomes.

    Provides leadership in patient education programs through the initiation of ideas, joint planning, problem solving, and evaluation of programs.

    Advocates for the integration of chronic disease prevention and health promotion initiatives in the planning and development of activities and programs within Maniilaq and in collaboration with other community agencies.

    Collaborates with the Maniilaq service area and statewide school system to provide diabetes prevention activates

    Collaborates with other service providers to plan and coordinate educational activities. Serves as liaison with local, state, regional, tribal, and federal/national agencies and organizations.

    Serves on OPD teams, wherever and whenever management determines the skills and talents of the incumbent would be best utilized; including but not limited to Improving Patient Care and Quality Improvement (QI) committees.

    Participates in developing prevention and promotion of SDPI, IHS, the Alaska State Diabetes Coalition and other state and national diabetes prevention frameworks and strategically plans the development of community strategies and local strategic partnerships.

    Represents the Diabetes Program and Maniilaq professionally and positively to enhance and promote the core values and mission of the company.

    • Prepares and submits monthly written and oral reports of service activities and other reports requested by contract or administration and meeting deadlines.
    • Prepare and submit quarterly, semi-annual, and annual reports within specified periods.
    • Establishes a working relationship with IHS SDPI project representative
    • Assists the Diabetes Program Assistant/Community Outreach Coordinator, or performs duties, as needed in their stead.
    • Leads activities appropriate to certification and participates in traditional seasonal activities including harvesting, preparation and presentation of traditional foods.
      • Health Fairs and diabetes prevention events that align with the SDPI Project Narrative for the calendar year
      • Diabetes screening booths at least twice a month

    Prepares flyers/advertisements, and plans/facilitates meetings

    Supports the Diabetes Program as needed:

      • Completes competency on glucometer blood glucose training and assists with glucometer teaching as needed
    • Completes competency on monofilament diabetic foot exam to assist program when needed
    • Applies for Alaska food card and complies with requirements if serving food during public events
    • Provides diabetes education and ongoing support the population served through in person, phone, or video telecommunication.
    • Assists with workflow demands such as referral management coordination and booking appointments as needed.

    Maintains a personal level of physical and spiritual wellness appropriate to serve a as a role model

    Required Experience

     

    • Associates degree in Nursing is required.
    • Must have a valid RN license.
    • Lifestyle coaching certification must be obtained within 6 months of hire.
    • Applicant must be self-motivated, able to work well on teams with minimal supervision and understand the fundamentals and concepts of public health and rural health organizations.
    • Experience with navigating electronic health records, telecommunication technology (using phones, computers, outlook calendar), preparing presentations and spreadsheets, and word processing programs is required.
    • Must be able to provide electronic, verbal and written communication with groups of people.
    • Able to travel in small planes.
    • BLS is required.
