    This position supports the Diabetes Program to ensure the effective delivery of comprehensive Diabetes Management and Medical Nutrition therapy to Maniilaq Service area. Assist in the management of patient services including overseeing the patient/member experience and the delivery of exceptional customer service for the diabetes and nutrition programs.

    This position is highly regarded as a frontline liaison between the community and the clinic. This position is expected to build effective partnerships with community providers that increases patient referrals. This job requires village travel and the formation of partnerships with village tribal councils and school administrators.   Reports directly to the Diabetes Assistant Program Manager who will maintain open communication with the Diabetes Program Manager on all staff concerns and program work flow.

    Required Skills

    PRINCIPAL DUTIES AND RESPONSIBILITIES  

    • Works with the Diabetes Program staff to coordinates Diabetes Prevention activities with the villages in the Maniilaq Service Area, Maniilaq Association, and other Alaska Native corporations, the Alaska Native Tribal Health Consortium, state and federal agencies, and other non-federal agencies and organizations. Assist in implementing other wellness activities including but not limited to: health screenings; health education seminars/ presentation, fitness classes, etc.
    • Assists with local and village diabetes screening booths that alternately and/or simultaneously offer SMBG checks, A1C checks, blood pressure monitoring and foot exams; making appropriate referrals as necessary.
    • Works within Cerner to schedule patients for education and support (PharmD consultations, Registered Dietitian and lifestyle coaching appointments) while ensuring that any needed labs are proposed, approved and drawn prior to appointment dates. Works within the referral que to ensure all emergent inpatient and Gestational Diabetes Mellitus (GDM) consultation request are scheduled appropriately.
    • Ensures daily diabetic surveillance reports (diabetic audits on OPD appointments) are printed out, reviewed and needed recommendations are noted. Ensures reports are uploaded into a shared drive utilized by the Diabetes Program for easy access of other team members and monthly quality auditing.
    • Performs nursing duties in the care of adult patients who have been diagnosed with diabetes and/or pre-diabetes. These duties include intake vital signs/documentation when necessary and basic diabetes support education that should include preventive as well as self-management skills. Performs yearly foot assessments and appropriately refers for additional care.
    • Maintains competency on glucometer technique and other devices used by the program. Provides clients with training of glucose monitoring technique using a glucometer. Uploading of meters and placing meter reports into the shared drive utilized by the Diabetes Program.
    • Assists diabetic beneficiaries to obtain diabetic foot ware (Drew Shoes) upon request. Orders and ensures delivery of Drew Shoes while maintaining accurate electronic documentation and invoices for billing purposes on the shared drive.
    • Becomes knowledgeable and understands the needs of diabetic patients while being able to provide empathetic care that meets them where they are in their journey of being diagnosed/living with diabetes.  Adjusts care to those in need, providing support and acceptance.  Patient interactions and Cerner documentation reflects this understanding.
    • Maintains and builds existing relationships with patients, community leaders, community based organizations, churches, schools, etc.
    • Assists the program to promote awareness of diabetes through a wide variety of activities. Participates in statewide diabetes prevention activities when called upon.
    • Assists with coordination of SDPI budgeted events that may include event giveaways and material needs, event resource requirements, and other expenses.
    • Works in coordination with the Program Manager and the Assistant Program Manager to develop and implement the Diabetes Program according to the program goals and objectives.
    • Supports and participates in teaching and training to expand services (i.e. basic foot care) and increase public awareness of diabetes prevention and control utilizing the Indian Health Service Best Practices and approved protocols.
    • Makes appointments and travel arrangements for clients as necessary.
    • Attends team conferences and networks for relationship building and resource development.
    • Engages in local travel as required and overnight stays to maximize travel opportunities.
    • Utilizes various computer programs, including Electronic Health Record (EHR), RPMS, Microsoft Office, Cerner and Access database programs.
    • Becomes knowledgeable (within their scope of practice) about diabetes care by utilizing the IHS website and the SDPI webinars in order to develop a basic understanding of the grant in which we provide diabetes care to our community. This position will be encouraged and supported in obtaining additional educational certifications within the diabetes care delivery system. 
    • Effectively communicates and works with program staff members and the interdisciplinary care team to continually support diabetes prevention and improved diabetes self-management care support.
    • Maintains professional ethical standards of conduct at all times.
    • Interacts in an effective manner with physicians, health team members, community agencies, and clients/families with diverse values, and religious and cultural ideals.
    • Maintains confidentiality and professional ethical standards of conduct at all times and is engaged in Maniilaq’s Customer Service Model
    • Works varied hours as needed.
    • Complies with Health Insurance Portability and Accountability Act (HIPAA)
    • Safeguards the patient’s right to privacy by judiciously protecting information of a confidential nature.

    Required Experience

     

    MINIMUM REQUIREMENTS

     

    Graduation from an accredited LPN program. Must have LPN license in good standing and be eligible to apply for an Alaska licensure as a Licensed Practical Nurse.  Must have knowledge of diabetes health issues, prevention education/ counseling, social services, behavioral health and strategies for community-based intervention and/or a general knowledge of health assessment, prevention, promotion and intervention methods are required.  Valid Alaska Driver’s License, or obtain within 6 months of hire.  Ability to work autonomously with little direction and self-motivated.  Demonstrates leadership qualities including time management, verbal, written communication skills, problem solving, decision making, and priority setting. May be exposed to communicable diseases and hazardous materials. Must be able to pass the core competencies assigned to the unit annually
