Job Description

• Assists the Director of Social Services in developing, implementing, and maintaining Quality Assurance programs for Social Services.
• Develops and maintains professional relationships with staff, physicians, residents, families, community resources, and third-party payers.
• Completes Admission forms/assessments and documentation as outlined in the Social Service Admission Process.
• Completes a plan of care for each resident based on their needs. Documents on progress towards care plan goal, and updates POC with significant change and/or quarterly.
• Assess residents various health needs on an on-going basis. Assess the need for referrals to health services and provides education and support.

Qualifications

• Bachelor of Social Work (BSW) required
• Experience in Long-Term Care or working with seniors required
• Comfort and experience working with dementia populations
• Flexibility to assist in our skilled, assisted, and residential living areas
