OUTREACH COORDINATOR:
SUMMARY
Outreach services are provided through a formal outreach program to both the community at large and to individuals. Services include education of the public and risk groups (mental health and/or substance abusing individuals), identification and linkage with high risk groups, planning and linking with other service providers, risks reduction and intervention.
ESSENTIAL FUNCTIONS:
Supervises outreach workers to ensure the completion of organizational benchmarks of engagement and productivity.
Maintains outreach workers informed on all accepted insurance plans and changes to the operational processes of the health center.
In coordination with administration, the outreach coordinator will forge new relationships and corresponding logistical workflows with referral sources to enhance continuity of care.
Educates referral sources on the services provided at the health center and the accepted methods of payment and in network third party payers.
Coordinates with the Clinical Operations Manager participation in health fairs to engage the general public on the provision of services afforded by the health center.
Collaborates with administration the execution of the organization’s marketing strategy.
Serves as a liaison for potential patients by facilitating future appointments that are most convenient to their schedules.
Develops methods for feedback from outreach locations and evaluates the program’s success, making adjustments as appropriate.
Prepares reports to management on outreach efforts.
Participate in staff meetings as scheduled.
Performs other duties as assigned.

EDUCATION AND/OR EXPERIENCE: High School Diploma required. Associate’s Degree preferred. Candidate will have a minimum of two years of supervisory experience with outreach and/or marketing teams.
LANGUAGE SKILLS: Ability to read, write and speak in ENGLISH AND SPANISH (or other languages) helpful. Must be able to read and comprehend simple instructions, short correspondence, and memorandums. Agility to effectively communicate information both orally and written on one-to-one and small group situations to clients and other employees.
MATHEMATICAL SKILLS: Ability to add, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio and percent.
REASONING ABILITY: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram or schedule form.
ORGANIZATIONAL SKILLS: Must be organized and able to schedule work load appropriately without hindrance to client appointments etc.
DECISION MAKING: Must be able to work independently with competent decision making using established criteria and guidelines.
OTHER SKILLS AND ABILITIES: Computer Skills: To perform this job successfully, an individual should have knowledge of Accounting software; Database software; Internet software and Word Processing software.
